Supervisor Best Practices

Supervisors are the front-line reporters of work-related injuries. Therefore, they have a primary responsibility to ensure employees perform post-injury response procedures in accordance with workers compensation policies. Compile a post-injury package consisting of:

1- Work Ability Form,   
2- Witness Report Form,
3- Supervisor Report of Incident, and
4- Employee Report of Incident

 
These forms should have a diagram of the front and back of the body so witnesses and injured employee can CIRCLE the injured areas of the body. Don't forget the diagram!
