In this issue we will present the papers presented at the ECU Conference in Dublin Ireland, review the recent 2014 SOT Research Conference, discuss what constitutes authorship, present new research of CSF production and circulation, and share how an occipital adjustment may help patients with a frozen shoulder.

ECU Conference 2014 The following is shared by Dr. Aimee Carroll our SOT representative to the 2014 ECU Convention:

"With a theme of 'celebrating diversity' the European Chiropractors Union Annual Convention, was held in the amazing Dublin Convention Centre. Speakers including, Bruce Lipton, Gerry Clum, Richard Brown, and Frederick Carrick, helped to make it a very memorable experience. Having four of the fourteen poster presentations being from SOTO-USA, and the only ones on SOT technique, it was a good showing for the organization, and many attendees showed interest. Moving forward, especially with trying to "celebrate diversity" in our profession, including in our techniques, I believe having more SOT research will be important!"

Carroll A, Blum CL, Young M. Treatment of low back pain in pregnancy with Sacro Occipital Technique (SOT): A case report.

Clinical Application: The purpose of this paper was to provide information of a potential way of treating pregnant women with low back pain and sacroiliac joint hypermobility, using Sacro-Occipital Technique (SOT) category two pelvic block placement.


Blum CL, Griffiths RL. Chiropractic and Dentistry- The Need for Mutual Understanding of TMD Co-treatment: A Case Report.

Clinical Application: How chiropractors and dentists can work together to help complex patient presentations is elucidated in this paper illustrated by a patient presenting with a history of orthodontic care and chronic TMJ disorders.


Moeller JL, Blum CL. Orofacial myofunctional therapy: A Case Report. J Chiropr Educ. Spr 2014;28(1):94.

Clinical Application: Orofacial myofunctional therapy (OMT) is a method performed by those trained to assess and manage an exercise program for patients, particularly with temporomandibular joint or stomatognathic disorders. Working closely with a chiropractor, a OMT therapist can teach specific exercises to help maintain a lip seal, improve tongue position, proper swallowing, and other important functional activites to facilitate interdisciplinary care of complex TMD or obstructive sleep apnea patients.


Evans A, Yelverton C. The validity and specificity of the arm fossa test.

Clinical Application: The arm fossa test is a test that is integral for the pre-assessment, length of treatment, and post-assessment of the patient being treated for SOT category two sacroiliac joint hypermobility complex syndromes. This study looks deeply into the evidence base of information about the arm fossa test.

Authorship


 What does it mean to be an "author?"



"Why is authorship attribution important? Aside from the fundamental issues of honesty, ethics, and integrity, correctly listing authors allows general readers and scientists to know the appropriate attribution of the publication to a particular author.1,2 Without appropriate attribution, it remains uncertain who assumes responsibility for the work published."

The International Committee of Medical Journal Editors recommends that authorship be based on the following 4 criteria:

  1. Substantial contributions to the conception or design of the work; or the acquisition, analysis, or interpretation of data for the work; AND

  2. Drafting the work or revising it critically for important intellectual content; AND

  3. Final approval of the version to be published; AND

  4. Agreement to be accountable for all aspects of the work in ensuring that questions related to the accuracy or integrity of any part of the work are appropriately investigated and resolved.

 A new look at cerebrospinal fluid circulation

While this fascinating study may not change the application of our cranial techniques, it does change how we conceptualize what we are doing with our cranial adjustments. Instead of the chorid plexus being where the CSF is formed and the arachnoid granulations where the CSF drains into the venous sinuses, according to this study we have a more generalized and dynamic situation.


"The CSF circulation comprises not only a directed flow of CSF, but in addition a pulsatile to and fro movement throughout the entire brain with local fluid exchange between blood, interstitial fluid, and CSF. Astrocytes, aquaporins, and other membrane transporters are key elements in brain water and CSF homeostasis. A continuous bidirectional fluid exchange at the blood brain barrier produces flow rates, which exceed the choroidal CSF production rate by far. "

The circulation and dynamic quality of the production and reabsorption through the Virchow Robbins spaces in the spine makes the central canal's circulation of CSF no longer insignificant and further explains what we are doing with our vasomotor category one procedures. The formation of local syrinx's at the vertebral level around traumatized spinal blood vessels traversing the spinal levels from the subarachnoid space to the central canal suggests a compelling reason for our category one procedures.

"The CSF circulation around blood vessels penetrating from the subarachnoid space into the Virchow Robin spaces provides both a drainage pathway for the clearance of waste molecules from the brain and a site for the interaction of the systemic immune system with that of the brain. "

I think as we digest this new article we will further understand the great work of our founder M. B. DeJarnette. His genius and insight just seems to keep growing as we better understand anatomy and physiology of the human body.

OTZ Tension Adjustment - Frozen Shoulder


 Murphy FX, Hall MW, D'Amico L, Jensen AM. J Chiropr Med. 2012 Dec;11(4):267-72.

OZT Frozen Shoulder "It is theorized that the OTZ Tension Adjustment identifies aberrant alignment of the occipitoatlantal articulation through visual inspection and motion palpation. In the procedure, the doctor is seated at the head of the supine patient and first performs a specific skull glide to determine the exact orientation of the dysfunctional joint. A high-velocity, low-amplitude thrust is delivered at the level of the dysfunctional C0-C1 joint into the direction of maximal restriction. The general line of drive is posterior to anterior, lateral to medial, and slightly superior to inferior. After the adjustment is made, the skull glide palpation is performed again to confirm correction of the dysfunction."

While the occipital adjustment isn't anything new, those involved with SOT for years can remember DeJarnette's Sacro Occipital Seminar Notes 1965 (pages 11-12) where the "occipital side-slip subluxation" was clearly defined and its adjustment described. But what wasn't discussed was its relationship to a "frozen shoulder."

In the Murphy et al study they found by adjusting the occiput "of the 50 cases, 16 resolved completely, regaining 180� of pain-free active shoulder abduction. Another 25 showed 75% to 90% improvement in active abduction, 8 showed 50% to 75% improvement, and 1 showed 0% to 50% improvement."
