In this issue we will share the SOT related studies that will be shared at upcoming Research Conferences. Also a recent study discussing SOT blocking as part of a treatment protocol for piriformis muscle syndrome, generalized hypermobility syndrome - its relationship to category two, and the limited research supporting the use of parachutes.

International Research and Philosophy Symposium


 October 20-21, 2012 - Sherman College, Spartanburg, SC

IRAPS This year we are honored to have four SOT related studies presented at this years International Research and Philosophy Symposium at Sherman College of Chiropractic. The presenters will be Drs. SOT Related Piriformis Muscle Study


 Journal of Chiropractic Medicine

Piriformis Muscle Chapman C, Bakkum BW. Chiropractic management of a US Army veteran with low back pain and piriformis syndrome complicated by an anatomical anomaly of the piriformis muscle: a case study. J Chiropr Med. 2012 Mar;11(1):24-9.

The purpose of this article is to present the case of a patient with an anatomical anomaly of the piriformis muscle who had a piriformis syndrome and was managed with chiropractic care.

Chiropractic treatment included lumbar and sacral spinal manipulation with soft tissue massage to associated musculature and home exercise recommendations. Variations from routine care included proprioceptive neuromuscular facilitation stretches, electric muscle stimulation, acupressure point stimulation, Sacro Occipital Technique pelvic blocking, CranioSacral therapy, and an ergonomic evaluation.

SOT and Hypermobility Syndromes


 Does Your Patient have Generalized Hypermobility Syndrome?

SOT Graphic In the article by Richard Gerardo, et al at the recent SOT Research Conference entitled, "Chiropractic and dental care of a patient with temporomandibular and sacroiliac joint hypermobility: A case report." a presentation about generalized hypermobility syndrome accompanied the paper.

The study was by Hakim and Grahame and published in the April 2003 International Journal of Clinical Practice, and was entitled, "A simple questionnaire to detect hypermobility: an adjunct to the assessment of patients with diffuse musculoskeletal pain."

Their simple and reproducible self-reporting questionnaire helps identify individuals with hypermobility. Analysis demonstrated that a positive answer to any two questions in this five-part questionnaire gave the highest combined sensitivity and specificity for detecting hypermobility. The sensitivity and specificity was 84% and 80-89% , and overall the questionnaire correctly identified 84% of all cases and controls.

2. Can you now [or could you ever] bend your thumb to touch your forearm?

3. As a child, did you amuse your friends by contorting your body into strange shapes or could you do the splits?

4. As a child or teenager, did your kneecap or shoulder dislocate on more than one occasion?

5. Do you consider yourself "double-jointed"?

Parachutes - Where is the Evidence?


 The Value of Observational Studies

parachute How many times have we heard that there isn't sufficient research or evidence for chiropractic or our SOT care? Often when our research is shared the next comment is it doesn't mean anything if they aren't randomized controlled trial (RCT) studies.

RCTs commonly use a control group who do not received treatment, a sham or placebo intervention to compare to the actual treatment, and optimally everyone is blinded so no one knows who is getting which treatment. These are great with studying medications since everyone can get the same blue pill and the interventions appear the same to everyone.

The problem with manual therapy such as chiropractic is that it is very hard to produce a chiropractic treatment that doesn't have some effect to a patient. Also isolating one aspect of a chiropractic treatment and studying that in isolation, which is needed with an RCT, often misses out on the whole body treatment needed for an effective intervention.

So it is not uncommon in manual manipulative health studies to utilize observational studies. Concato, Shah, and Horwitz, found in a New England Journal of Medicine study entitled, "Randomized, controlled trials, observational studies, and the hierarchy of research designs." that "...well-designed observational studies (with either a cohort or a case-control design) do not systematically overestimate the magnitude of the effects of treatment as compared with those in randomized, controlled trials on the same topic."

So with that backdrop I highly recommend that you take a moment to read this article published in the British Medical Journal, entitled, "Parachute use to prevent death and major trauma related to gravitational challenge: systematic review of randomised controlled trials"

It is humorous to note that they conclude, "As with many interventions intended to prevent ill health, the effectiveness of parachutes has not been subjected to rigorous evaluation by using randomised controlled trials. Advocates of evidence based medicine have criticised the adoption of interventions evaluated by using only observational data. We think that everyone might benefit if the most radical protagonists of evidence based medicine organised and participated in a double blind, randomised, placebo controlled, crossover trial of the parachute."

SOT at the ACC RAC Conferences 2013


 Getting Ready for March 2013 in Washington DC

ACC RAC 2013 Once again SOTO-USA has submitted a series of SOT related articles to this prestigious annual event. SOTO-USA has had a paper accepted each year at this annual conference since the ACC joined the RAC in 2001. In the next newsletter we hope to share with you a list of those papers accepted for the conference in 2013.

Many of the papers submitted were formulated and shared at the SOT Research Conference which is given prior to the SOTO-USA Clinical Symposium in May. Please consider submitting a paper to this years SOT Research Conference, click here.

Sacro Occipital Technique Organization - USA is a 501c3 non-profit, professional organization formed to promote the awareness, understanding and utilization of the Sacro Occipital Technique method of chiropractic as founded and developed by Dr. Major Bertrand DeJarnette.
