Welcome to the Aging Well Digest.


In this issue you will find articles on 1) the work of Friends of Residents in Long Term Care, a NC policy not for profit that advocates for long term care residents, 2) Medicare's new evaluation measures for hospital patient satisfaction, 3) the Green House Movement, a progressive method for delivering elder care, and 4) the severe shortage of Geriatric physicians. 

I trust you will be able to find something useful, informative, or interesting in this issue. Friends of Residents of Long Term Care

     Friends of Residents in Long Term Care is a nonprofit that for twenty five years has been committed to advancing the quality of life in North Carolina's long term care facilities by advocating for changes in public policy, educating and supporting families of long term care residents, and building public awareness about LTC issues. It represents over 90,000 people in our state who can no longer live independently. There is really no equivalent organization in North Carolina that exists solely to lobby for long term care residents and it is often up against a variety of well funded LTC industry organizations who have the luxury of full time lobbyists. 
       I mention this now because "Friends" is having its once a year fundraising gala on November 19th at the NCSU University Club in Raleigh and the funds it raises there are critical to its mission. In a state that AARP and the Commonwealth Fund ranked 45th in the nation for quality of care, I can think of no other place where your money would be better spent in the effort to improve that ranking. 
      Please consider purchasing a ticket for the Gala at www.FORLTC.org (Sec of State Elaine Marshall will be the speaker) and if your schedule doesn't permit, I hope you will give a donation to the organization through that website. 
 
 
 

Patient Satisfaction and Medicare Reimbursement

 

 canada lakeA recent New York Times story describes Medicare's new evaluation process for reimbursing hospitals that takes into consideration patient satisfaction scores. Hospital satisfaction ratings will be based on questions like, "Did doctors and nurses communicate well?", "Was pain well controlled?", "Was your room clean?" and " Was it quiet at night?" and the surveys will include younger people as well as Medicare beneficiaries. Some hospitals claim this comparison is unfair but nevertheless it illustrates the clout a consumer has as a hospital patient. Your satisfaction is directly connected to the hospital's bottom line. Take a look at the Health and Human services site linked below to see how your hospital has scored in previous years. Put in your zip, mark several hospitals, hit search, and look for Patient's Hospital Experiences

De-Institutionalizing the Nursing Home

 

topsail cloudsIn 1987 the Nursing Home Reform Act declared that residents in long-term care have the right to be free from abuse and neglect. Since that time a variety of strategies have been tried to make nursing homes more"resident centered" and none have been more successful than the Green House movement. It is a comprehensive effort to reinvent the conventional nursing home experience, including its physical appearance and the way medical care is delivered. While there are only just over one hundred of these facilities in the country, their model exerts pressure on the industry to engage in similar transformation and may very well be a peek at the future of aging.

 Where are the Geriatricians?

 beach seaweed A recent study of pain medication delivery in Hospital Emergency Departments finds that patients 75 years old and up received significantly less medication for severe pain than those in the 35-54 range. The reasons for this aren't entirely clear but many suggest that ED physicians may be more concerned about negative side effects in this population. This begs the question, "Where are the geriatricians who might give qualified direction in such circumstances?"

     Unfortunately, the elderly are often cared for by physicians who have no special training in geriatric care. Nor are they likely to be soon. According to the American Geriatrics Society, there is one geriatrician for every 2600 Americans over 75 and by 2030 that ratio will be one for every 3800. The new federal health overhaul includes $29 million to fund a range of geriatric care programs but unfortunately the shortage will continue for some time.
