SMI Eligibility Determination Program

As you are aware, on May 14, 2013, the Arizona Department of Health Services awarded the Crisis Response Network, Inc. a contract to establish and administer the Seriously Mentally Ill (SMI) Eligibility Determination Program for Maricopa County. Effective January 1, 2014, Crisis Response Network will assume responsibility for operating the Program from Magellan Health Services. Both the Crisis Response Network and Magellan are committed to making this transition as seamless as possible.

 

As part of this transition, Magellan will continue to carry out the following duties:

  • Accept referrals for SMI evaluations through Magellan Customer Service at (800) 564-5465
  • Coordinate SMI evaluations through Crisis Preparation & Recovery or other contracted providers
  • Assign individuals determined to be SMI to an appropriate clinic to receive behavioral health services

 

The Crisis Response Network will accept completed SMI assessment packets from evaluating providers through the following methods:

  • Crisis Response Network Web Portal at www.crisisnetwork.org/smi/
  • Fax at (888) 656-2659
  • Standard mail at: Crisis Response Network, 1275 W. Washington Ave., Suite 102, Tempe, AZ 85281

Attend Magellan's Shared Governance Board Meeting Wednesday, Dec. 18!

You're invited to join Magellan Health Services of Arizona's Shared Governance Board for their bi-monthly meeting tonight, Wednesday, Dec. 18, from 5 to 7 p.m., at Valle Del Sol located at 3807 N. 7th St., Phoenix, AZ 85014. Board meetings are open to the public.

 

At the meeting, the Board will receive an update from Valle del Sol's Chief Executive Officer Kurt Sheppard who will cover their key initiatives and organizational goals. The Board will also discuss updates on Magellan Health Services of Arizona key initiatives and strategies.

  

We encourage attendance from provider staff, recipients, family members and the community. This is a great opportunity to observe the Board in action and learn more about what is happening in central Arizona's behavioral health system of care. In addition, Board meetings also provide those in attendance with the chance to provide input and ask questions during the public comment portion of the meeting. There is a two-minute speaking limit to ensure everyone who wishes to speak has the opportunity to ask questions or provide comments (please review the Governance Board FAQ for more details).

NAMI Family-to-Family

Family-to-Family is a 12-week course, taught by trained family members, for family caregivers of individuals with severe mental illnesses. Click HERE to learn more about the course in 2014.

-- Treating Tobacco in Clinical Settings, Monday, Jan. 13, from 1 to 2 p.m.

-- The Bridge, Tuesday, Jan. 14, from 9 a.m. to 12 p.m.

-- Arizona State Hospital, Tuesday, Jan. 14, from 10:30 to 11:30 a.m.

-- Co-Occurring Disorders, Monday, Jan. 20, from 9 a.m. to 4 p.m
