Subject: Medication Formulary and Prior Authorization Updates

Dear Provider,

Magellan has several pharmacy updates we would like to bring to your attention. DBHS has developed a new state-wide Behavioral Health Drug List (BHDL). There are many drugs that have been added to the formulary, some with a prior authorization and some with no prior authorization if used at FDA approved doses. There are also several new clinical prior authorizations for antidepressant and antipsychotic combinations.

The new formulary additions are effective January 1, 2014. All of the Prior Authorization and formulary documents are attached below and are also available via our Magellan Pharmacy website at: http://www.magellanofaz.com/for-providers/pharmacy.aspx
