AHA NOVA Award Winner: PSPH Emergency Center ProgramPelllicer 

   

The American Hospital Association (AHA) announced last week that the Emergency Department Consistent Care Program at Providence St. Peter Hospital is one of five programs across the country to receive the 2011 AHA NOVA Award. The award recognizes hospitals and health systems for their collaborative efforts toward improving community health.  The Providence St. Peter Foundation provided a $28,000 grant in 2006 for support and evaluation of the program.  Ken Anderson, the foundation's board president, says, "We are proud that charitable gifts made a difference in changing health care."

  

The Consistent Care program is an intervention program that guides treatment decisions for frequent emergency department patients with complex health needs. The majority of these individuals have issues regarding mental health, chemical dependency and/or unmanaged chronic conditions.


Medrice Coluccio, CEO of Providence St. Peter Hospital, said, "We are honored to receive this award from the American Hospital Association. The Consistent Care program represents an interdisciplinary approach of providing vulnerable members of our community with hope and resources to assist in improving their health."   

 

To date, Consistent Care has assisted more than 630 patients, expanded to include four other hospitals in a five-county region, dramatically reduced emergency visits, and saved millions of dollars in charges. The program is operated in collaboration with CHOICE Regional Health Network - a non-profit community-based collaborative care network of rural and urban hospitals, practitioners, public health clinics, community health centers, behavioral health programs and others.

"Funding from Providence St. Peter Foundation came at an important time and provided the resources to do a formal evaluation of the program.  We consider the foundation's investment critical in taking this best practice from pilot to sustainable program" said Kristen West, Executive Director of CHOICE Regional Health Network.

Gifts enable Foundation to award $259,578 in grants  

 

Thanks to charitable gifts from generous individuals, businesses and

organizations, the Providence St. Peter

Foundation Board

approved $259,578 in funding in the spring grant cycle for Providence facilities and services.

 

Included in the funded grants was $54,642 to purchase

a Quinton Monitoring System to track

the progress of the ever increasing number of cardiac rehabilitation patients at the hospital.

 

The new upgrade will mean more flexibility for staff and patients in starting the day.  With all the monitors networked, it will simplify patients being assigned monitors each day and will then free up staff to take care of other patients.  These new systems will also allow staff additional access locations to chart on each patient and to track outcomes; increasing productivity and efficiency.

Overall in the last four years, the St. Peter Foundation has allocated more than $11.5 million to Providence programs and services locally, including grant awards and other charitable distributions.
