This Hi-Tech Hi-Five addresses new MDS 3.0 HIPPS codes billing, 5010 testing, End of Year processing, and EMR features that support your facility's falls prevention efforts.

 

Hi-One: New HIPPS Codes End with A, B, C and D

Effective 10/1/11 the MDS will generate new HIPPS codes that end in the letters A, B, C or D. 

 

These codes are part of the two-character Assessment Indicator (AI) that identifies the type of assessment. The second AI character indicates an unscheduled assessment used for PPS. The new codes (A, B, C, D) identify new assessment types, including the COT and EOT-R, that came into use on 10/1/11.

 

Hi-Tech MDS error checking accommodates these new codes. If you receive any errors on these codes within the Hi-Tech system, please download Patches from our website.

 

CMS is adjusting its error checking to accept claims that use these codes. If any of your claims are rejected due to these HIPPS codes, call Medicare to confirm the reason. Rebill these claims when Medicare is ready to accept the new codes.

As soon as possible, test 5010 claims submissions for Medicare A and B so you can transition into production claims. This will help you avoid a last-minute rush at the end of the year.

We will provide instructions for testing Medicaid 5010 claims at a later date.

 

 

Hi-Three: Use Hi-Tech EMR Documentation to Record Residents' Falls

According to the CDC, nursing home residents fall at twice the rate of older adults living in the community.

 

Hi-Tech EMR Documentation provides several tools that will help you address residents' falls in your facility. For example, you can use the Activities/Events module to record Falls incidences.

 

First, under Libraries > Documentation > Activities/Events, set up a Falls record and checkmark Satisfies MDS J1800 (3.0) and/or Satisfies RCA Section J-7 a/b (for Maine Residential Care).

 

When a resident falls, record it under Documentation > Activities / Events. From this record you can access the resident's Notes to provide additional details about the Fall. Use Events Reporting to list and review recorded falls.

 

When you record falls information at MDS 3.0 Section J1800, click Pull J1800 response from Activities/Events Detail to display a list of the resident's recorded falls so you can answer J1800 Any Falls Since Admission or Prior Assessment.

 

When you record falls information at RCA J7 a and b, click Pull J-7 a/b responses from Activities Detail to automatically complete those responses using recorded falls information.

 
Hi-Four: Identify Residents at Risk for Falls

In addition to a resident's falls history, the MDS 3.0 serves as a record of several falls risk factors, including cognitive impairment, vision impairment, incontinence, psychoactive medications, balance during transfer and walking, specific diagnoses, and use of restraints. The Hi-Tech QI/QA/Survey Reports module helps you access this information.

 

QI/QA/Survey Reports > Quality Assurance > MDS: Search for residents whose MDS includes specific responses or combinations of responses; for example, review a list of residents who are Unsteady during transitions and walking (G0300 A-E) and/or receive psychoactive meds (N0400 A-D). Save your report designs so you can frequently review this information based on new assessments.

 

QI/QA/Survey Reports > Survey > CMS 802 and CMS 672: These survey reports are based on each resident's current MDS record. The CMS 802 Roster lists residents whose MDS responses cause them to be included in the Falls category and other categories related to a falls risk factor:

  • 06. Falls
  • 10. 9 or More Falls
  • 11. Cognitive Impairment
  • 12. Incontinence
  • 22. Psychoactive Medications
  • 24. Activities
  • 28. Vision/Hearing/Devices

You can also review your residents' medication orders for med types that could contribute to falls.

 

In the Hi-Tech system, under Admission Procedures > Edit Orders, as you enter med orders, assign psychoactive meds to four different drug classes, such as antipsychotics, antidepressants, antianxiety, and hypnotics. You can then use QI / QA / Survey Reports > Quality Assurance > Medications to list residents who receive meds in these psychoactive drug classes.

 

From the IMAR Electronic Medication Administration system, select Reports > Therapeutic Class Report to list residents on Psychoactive meds.

 

Hi-FiveDecrease Your Residents' Falls Incidences

Use the following tools to assess the resident and to implement procedures to avoid falls:

 

Documentation > Clinical Assessments: Complete template 01 Risk for Falls to determine that a resident has a low or high risk for falls. Complete the assessment as often as necessary and review changes and trends in level of risk.

 

MDS/Care Plans > MDS > Edit CAA Resources: Several MDS responses indicate falls risk factors that will trigger the Falls Care Area. Completing the Falls CAA helps you assess a resident's risk for falls to determine if you will care plan the resident for falls.

 

MDS/Care Plans > Care Plans: Add falls-related problems, goals and approaches to a resident's care plan.

 

Documentation > Activities / Events: Record resident's participation in exercise programs designed to improve mobility and reduce risk for falls.

 

Human Resources > Employee Tracking > Education: Track staff participation in fall prevention training.
