Congratulations! You are successfully navigating October 1 changes to the MDS 3.0, which included a revised PPS Assessment schedule and the new Change of Therapy (COT) and End of Therapy Resumption (EOT-R) forms.

 

Please remember to use the following programs as you implement these changes:

  • MDS/Care Plans > Scheduling / Pre-Asmnt >
     
    • Medicaretrack the scheduling and completion of MDS required for PPS billing. Note that COT assessments with ARDs from the 1st through the 6th could affect RUGs and payment for days in the previous month. Review each resident with a COT assessment and print the Billing RUG Changes Report for more information.
       
    • COT Assessment Alert: lists residents whose therapy has changed enough to warrant a COT (requires input through the Rehab Therapy System).
       
  • Reporting > MDS > Billing RUG Changes: MDS RUG changes that you should give to your billing department.

Hi-One: Check MDS Responses for Discrepancies

MDS Data Conflict Checking locates MDS responses that seem to conflict with each other.

 

For example: I5800 Depression (checked) and N0400 C. Antidepressant (unchecked).

 

The program checks for more than 100 conflicting responses that CMS would not consider to be errors, but which might attract a surveyor's attention, affect your case mix index, or become part of your profile on the Nursing Home Compare website.

 

Run the Data Conflict Check from three locations (see the menu below):

  • Edit MDS when you end out of an MDS record.
  • Print MDS when you select pages to be printed.
  • Data Conflict Checking from the menu to check a range of selected MDS records.

Hi-Two: Review your Survey Reports before your Surveyors

Like the CMS Survey reports provided to your surveyors, the Hi-Tech System reads your residents' MDS 3.0 records and compiles your own in-house copy of the CMS 802 Roster and CMS 672 Census and Conditions.

  • Review these reports for a specific MDS record to identify the responses that will include the resident in an 802 or 672 category.
  • Compile the survey reports for the entire facility at any time to identify and resolve situations before your surveyor arrives. 

 

Hi-Three: Compare your Residents' Billing and MDS Diagnoses

Through Edit Medical Record > Diagnoses you can record up to 30 ICD9 codes in a resident's face sheet record. These ICD9 codes become part of the resident's Stay Record and are submitted with Medicaid and Medicare billing.

 

You also select diagnoses in Edit MDS, Section I, and these are submitted with the MDS record.

 

To verify that you are submitting the same information in both records, select Reporting > MDS > MDS/Billing Diagnosis Review to list and compare the recorded diagnoses. See the sample below.

Do you ever find yourself thinking, "I wish I could get a list of MDS ...." ? MDS Reporting

 

You probably can, and we urge you to explore each of the options on the Reporting > MDS menu (shown on right).

 

You can also select QI/QA/Survey Reports > Quality Assurance > MDS to design reports that identify residents with specific MDS responses or combinations of responses; for example, residents on antipsychotic meds who have had falls. Save your report designs for regular reporting.

 

Also on the Quality Assurance menu is the Unresolved Conditions reporting program. This report compares residents' two most recent MDS records and lists conditions reported on both, including verbally and physically abusive, socially inappropriate behavior, burns, surgical wounds, open lesions, pneumonia, septicemia, UTI, fever, vomiting, weight loss, insufficient fluid; dehydrated and parenteral/IV. 

 

 

 

Hi-FiveStore MDS Chart Copies Electronically

If your facility has an Electronic Medical Records (EMR) Policy and a fool-proof backup system, you can maintain just the electronic copies of your residents' MDS 3.0 records. Rather than print the entire MDS 3.0 for the chart, print just the required Signature pages for the chart.

  • You must thoroughly understand CMS' criteria of the EMR policy in relationship to the MDS 3.0. To learn more about this, contact your state Help Desk.
  • Backup your MDS 3.0 files at least once a day to guard against loss of this important electronic information.

IMPORTANT: Hi-Tech is not responsible for lost MDS records if your system fails and you do not have backup copies.
