New Adverse Action Notices for MA Auto and Home We are required by law to notify policyholders if loss history or out-of-state driving records gathered from a third-party source could potentially impact new business premium. An Adverse Action Notice will be included with the insured's New Business package when applicable. For Homeowners, Form 11AR1292 and for Auto, Form 11AR1283. Both forms will be available in the Personal Lines Forms Library on arbella.com. No other action is required by the insured or agent.
