Medicare and Medicaid 50th Anniversary Countdown 
 

This summer marks the 50th anniversary of the enactment of amendments to the Social Security Act that established the Medicare and Medicaid programs.

Welcome to the Smoky Mountain LME/MCO Provider Network Bulletin. This monthly bulletin features news you need to know as a provider in the Smoky network and information you need to do business with Smoky.
  
Smoky manages public funds for mental health, substance use and intellectual or developmental disability services in 23 counties in western North Carolina. It is our pleasure to work with you to serve the needs of our local residents.

Last month, I invited you to revisit the values you hold dear and how they compare to those guiding the public system. As I noted, we often face challenges in practicing those values on a daily basis.

  

One challenge involves moving from an unmanaged fee-for-service system to managed care. Smoky CEO Brian Ingraham provided an account of this change and what it means to providers in July's "News You Can Use" newsletter.

 

Smoky's Selection and Retention Policy offers a fair, impartial and consistent process. One might say, "Of course Smoky thinks that, Smoky wrote the policy!" If you are a provider affected by recent decisions or purely an observer, you may disagree, question our authority or be fearful. My goal is not to ask you to agree with Smoky's decisions. It's about asking you to understand the direction we're taking in a more informed manner.

 

Smoky has been consistent in recent years in moving toward a comprehensive care center (CCC) model. One of our most important values is to serve the uninsured or underinsured. We do not have the luxury to NOT serve these individuals. Comprehensive care centers promote access to needed services for all people - not just individuals who receive Medicaid. Last year, Smoky served more than 17,000 people in 23 counties who were either uninsured or underinsured. The only way we can serve the thousands of people in need of care but not eligible for Medicaid is by creating a network model committed to serving the most people, regardless of funding source.   

 

Most providers are challenged in providing services to these individuals using current funding to cover expenses. I'm sure some of you serve the uninsured or underinsured based on your values, principles and moral compass. However, most providers cannot afford to take a loss and serve a large number of people with no payment source. The earlier, unmanaged system of care failed to hold providers accountable for providing services to those unable to pay. That system provided a false sense that these people were receiving care.

 

Smoky has been open and honest about our CCC model, the process for identifying comprehensive providers and what services they are required to provide. Under this model, many people who never received care are now offered services. The need for more services will increase as the number of individuals unable to pay for care and those living with mental health, intellectual or developmental disability or substance use challenges continue to grow. This is one reason we believe the CCC model is the best way to serve the most people.

 

This model is the "best thinking" to implement our values that all people are equal, and to maximize state funding for indigent care. If you have innovative ideas of how to serve more people without funding, I would love to hear from you.

Consumer Perception of Care Surveys
 

Smoky Mountain LME/MCO (Smoky) is grateful to the providers who assisted with the 2015 Perception of Care Survey last month. We implemented a new process this year involving randomly selected providers and hands-on administration of surveys. Thank you for welcoming Smoky team members into your facilities to conduct the surveys. Through your participation, we exceeded our goal of 822 surveys.

 

We should receive raw survey data from the NC Department of Mental Health, Developmental Disabilities and Substance Abuse Services by early October. Survey participants will be sent their data shortly thereafter.

Electronic Self-Assessment Launched for NC Home and Community Based Services Providers

  

The NC Department of Health and Human Services (DHHS) has launched the electronic self-assessment for Home and Community Based Services (HCBS) providers. The e-assessment must be completed by agencies that provide the following NC Innovations services:   

  • Day Supports
  • Residential Supports
  • Supported Employment

The e-assessment is also required for providers of Adult Day Health services offered through other Community Alternative Programs.

 

The e-assessment is mandatory and must be completed by September 15, 2015. Detailed instructions and the e-assessment can be found on the DHHS website. If you have questions about the e-assessment, please email hcbs@smokymountaincenter.com.

 
 
NC Council of Community Programs: 2015 Conference
  
  
The North Carolina Council of Community Programs will hold its annual conference and exhibition December 2 through 4, 2015 at Pinehurst Resort in Pinehurst, NC.

   

The conference is one of the largest in the state for leaders of the mental health, substance use and intellectual and developmental disability service system. For more details, see the conference brochure and sign up using the registration link.

Changes to State-funded personal care service array

  

Please be advised that State-funded personal Care (YM050) is no longer included in the available service array as it does not map to ADSN (Adult IDD) in the Department of Mental Health service array. Therefore, effective September 1, 2015, Smoky will not accept new requests for YM050 with an effective date of service September 1, 2015 or later. Please see the updated authorization guidelines illustrating removal of that service and increased intensity of Personal Assistance.

About the training:

The Provider Advisory Council of Smoky Mountain LME/MCO and OPTUM360 have partnered to provide an in-depth coding and billing training for providers. This training is a critical step in the transition from ICD-9 to ICD-10 processes.

 

Topics covered in the training include:
  • Benefits of ICD-10-CM and differences with the Diagnostic and Statistical Manual of Mental Disorders (DSM)
  • Guidelines, conventions and principles for key areas of focus: mental, intellectual and behavioral, nervous system, injury and poisoning and health status
  • Processes for appropriate code selection

Registration is required. Seating is limited to 130 attendees. Webinar access will be available (limited to 45 participants).
