Northeast Medica Project

NE 

A new initiative in the Northeast region is offering supportive housing for Minnesotans experiencing disabilities, health challenges, and long-term homelessness.  Aligned with our strategic goal to integrate health care systems with supportive housing, the new initiative is a collaboration among a health plan, social service providers, and Hearth Connection.

 

In partnership with Medica and the Human Development Center (HDC), this initiative will offer supportive housing designed to achieve housing stability, improved well-being, and decreased reliance on emergency and inpatient medical care.  As always, service plans will be flexible and participant-driven, with a focus on addressing health needs and access to appropriate care.

  

The new initiative will serve five people in St. Louis County and the Duluth area over the next several years.  During this period, Medica and Hearth Connection will track client-centered outcomes and offer progress reports.  We hope to continue pursuing similar initiatives to increase supportive housing opportunities throughout the state.

Minnesota Housing Invests $54 Million in Affordable Housing

Last Thursday, Minnesota Housing announced awards totaling $54 million for affordable housing development.  According to a press release from Minnesota Housing, the funding will create 1,904 units of affordable housing across Minnesota.  Click here to see the full list of funded projects.

 

Minnesota Achieves Strategy in State Plan to End Long-Term Homelessness

Along with the news of its recent funding awards, Minnesota Housing announced the achievement of Minnesota's original state plan to end homelessness.   Released in 2004, Minnesota's Business Plan to End Long-Term Homelessness set a strategy to create 4,000 housing opportunities for the state's most vulnerable and high-cost homeless population. 

 

A few months short of the plan's ten-year anniversary, the state has reached its mark with the funding of 4,027 long-term homeless housing opportunities.  Hearth Connection is proud of its role in the formation and fulfillment of the plan, and we remain dedicated to the goal of ending long-term homelessness in Minnesota.

The Search and Transition Committee is reviewing resumes of candidates for Hearth Connection's Executive Director position.  Applications are still being accepted.
