Elena finished 10th in the 69kg weight bracket and continues her quest to the 2016 Rio Olympics in search of Gold.

Physical Therapy diagnosis includes identifying the origin of an injury as
well the source of pain.
Physical Therapy Diagnosis vs. M.D. Diagnosis

In 2014, California finally joined the rest of the country in enacting a law that allows patients direct access to physical therapy services.

Before 2014, Californians were required to obtain a medical diagnosis from a medical doctor before they could seek physical therapy even though other states have had direct access laws for over 30 years. Obtaining a medical diagnosis before seeking physical therapy is not cost-effective or medically necessary.

An M.D. often relies on medical imaging and/or other expensive hi- tech diagnostic short cuts to reach a diagnosis. An experienced Physical Therapist does not need an x-ray, MRI, or lab test to diagnosis and treat the most common physical ailments. Tennis elbow, low back pain, ankle sprains, muscle strains and a stiff neck require only a careful examination by a P.T. to accurately diagnose the problem and treat it.

The uniquely American practice of engaging an Orthopedic Surgeon for common physical complaints compounds this waste of healthcare dollars. Ninety-nine percent of aches and pains do not have a surgical solution.

Physical therapists have advanced knowledge in kinesiology, anatomy, human biomechanics, pathomechanics, physiology, and the diseases that create physical impairment. With this specific knowledge, who better than a PT to evaluate and treat common physical ailments? We don't prescribe medicine, perform injections or do surgery. Rehabilitation is our specialty.

Why then, with all the talk of cutting health care costs over the last 20 years and the stacks of research showing direct access saves consumer dollars, did it take so long for California to join the PT revolution? Because the California Chapter of the American Medical Association fought the change tooth and nail in an unethical turf war.

Since 2014, PTs have rightfully assumed the role of primary care practitioners for common physical ailments and the cost savings have been substantial. Not only does direct access to physical therapy save time and money on the road to becoming pain-free, only a physical therapy diagnosis addresses the cause of the painful condition so it doesn't reoccur once you become active again.

An MD may diagnose a biceps tendonitis of the shoulder. A physical therapy exam also identifies the painful tendon but goes further to diagnose weakness in the muscles that stabilize the shoulder blade and the faulty mechanics responsible for the biceps irritation.

While a physician might inject the painful tendon or prescribe pain medication, a physical therapist could reduce the inflammation with less invasive treatments. More importantly, a PT would teach you how to rehabilitate the shoulder joint as a whole and prevent reoccurrence of the biceps tendonitis. Medical treatment typically reduces patients to passive recipients of care. Physical therapy requires active participation. You learn therapeutic exercises and ergonomic modifications that resolve your pain AND prevent the pain from returning.

Low Back Pain Guide

 Back Pain is a common occurrence for many Americans, but it is important to know when to take your symptoms more seriously and seek treatment from a physical therapist.  Without proper diagnosis and rehabilitation, seemingly minor aches and pains can turn into a much more serious and painful problem.
  
Daily activities like bending, twisting, lifting and sitting in poor posture create forces that damage the tissues and joints of the spinal column. Add in additional stress placed on the spine by the ill-advised exercise programs that are the norm in gyms across America, and it's a recipe for disaster. The truth is that the "core" exercises advocated by many fitness professionals have a high potential to damage the spine-- the very structure they are supposed to bolster and protect. 
 
Many popular "core" exercises designed to build a strong, flexible musculoskeletal system actually damage the spine. The damage is cumulative and may not become apparent immediately. When it comes to exercise and the spine, it is imperative that every position and motion be performed in such a way that the spine is protected.
 
Lumbar Disc Injuries
The most commonly injured spinal structure is the intervertebral disc (the shock absorber between the bony segments of the spine called vertebra). Bending, lifting and excessive sitting, especially in poorly supported postures, like sofas, soft overstuffed chairs, and slouching can all damage the intervertebral disc.
 
The constant pressure on the front of the disc created by excessive sitting and repetitive bending forces the syrupy fluid inside the disc to the back wall. Eventually this weakens the ligament that wraps around the disc and causes a bulge, much like a bubble on a tire tube. Common symptoms of a posterior bulging lumbar disc are: pain with forward bending, sitting, lifting, coughing or difficulty standing up straight after sitting. This may, or may not, be accompanied by pain shooting down the back of the leg. Numbness and tingling in the lower extremities is a less common but much more serious condition. Any of these symptoms indicates that a ruptured disc or boney growth(s) is compressing a nerve into the canal where the nerves travel as they come off the spinal cord. These symptoms need to be evaluated by an experienced Physical Therapist as soon as possible. Lower extremity weakness indicates an even more serious presentation of these spinal disorders and is an emergency that requires immediate evaluation by a PT.
 
Facet Joint Pain
The facet joints are in between the vertebra. They allow a vertebra to articulate with the vertebra above and below it.

Facet joints are prone to damage and arthritis. Bad posture, ill-advised trunk rotation and side bending exercises can all cause damage to them. Localized pain that is more intense on one side of the lower back and increases during extended periods of standing can indicate a facet joint problem. Facet joint pain may be worse when bending sideways or backwards and with rotation to that same side.

Joint Pain: A Reality of Life
  
You wake up in the morning and limp to the bathroom, as the sore knee that use to be stiff occasionally, is now painful almost every day. You vaguely remember an injury from your high school athletic days or maybe an old ski mishap, but as far back as you can recall it's been your "trick" knee. Maybe your doctor casually mentioned "arthritis" when you went to have it checked out a few years back but he didn't seem all that concerned and gave you no direction what you might do about it. Now it's getting worse and so is your concern.
 
This scenario is unfortunately too common as 27 million Americans suffer from osteoarthritis, a progressive degenerative joint disease characterized by breakdown of joint cartilage and associated with risk factors, such as previous joint injury, obesity and age. It's unfortunate because there was back then, and still now, something you can do about it. While it's true all of our joints wear as we age, and if they remain asymptomatic until you die you may never hear the diagnosis of arthritis, but if they become symptomatic and limit your ability to exercise and stay mobile there are far reaching ramifications for your overall health.
 
Osteoarthritis is specifically a breakdown of the cartilage that caps the end of our bones and it functions like a Teflon type material to prevent friction when the bones move on one another.  You can see it most clearly as the smooth white surfaces on the end of chicken bones. This material is constantly growing outward from the interface with the bone as the surface cells are sloughed off with normal use. However, a traumatic injury deep enough to impact the base layer from which they grow, or a chronic overuse of one particular area of the joint from altered joint mechanics causing accelerated wearing of the surface cells that your body cannot replace fast enough, results in a disrupted cartilage surface and an arthritic condition. Once disrupted, chronic inflammation plays a major role in accelerated joint cartilage breakdown.
 
If the American Academy of Orthopedic Surgeons and the American Physical Therapy Association did as well as the American Dental Association in getting us all to realize that our joints need daily health maintenance just like our teeth, there would a lot fewer disabled joints in our population and many less joint replacements each year. Over 1 million Americans receive joint replacements each year in the US and this number is projected to grow by over 600% by the year 2030. While this number is impacted by the bulging number of baby boomers entering the 6th, 7th and 8th decades of life it is also greatly impacted by their bulging waistlines.

Economy of motion 
In the 30 years Robert Forster has practiced sports physical therapy in Santa Monica, Calif., he has treated world record holders, injured athletes and grandmothers recovering from hip surgery.
"Our bodies have evolved to run," said Forster by telephone from London, where he was working with the U.S. Olympic Track and Field team. "Our tree-dwelling ancestors had shorter legs, longer arms and shorter feet. The body actually changed to be effective at running. But we lost our ability to co-opt that economy of motion."
Forster said the current physical capacity of the average Western civilian is about 10 percent of what is possible.
"Some people can run 100 miles in a day," he said.
The most common running mistake he sees involves stride length, or the distance of the foot on the ground.
"Everyone is over striding," he explained. "You want to land under your center of gravity, or as close to it as possible. We tend to take too few steps per minute. Less time on the ground would take care of a lot of problems."
For the runner seeking greater efficiency, Forster prefers a series of running-related drills -  among them arm swings that target the pendulum-like motion and knee-high drills, which he described as prancing "like a Clydesdale horse."

Phase IV Scientific Health and Performance Center works with professional athletes, laypersons, and everyone in between. Old or young, experienced or novice, Phase IV's methodical application of scientific training principles covers the gamut of sport-specific or endurance training, gym- or home-based functional strength programs, nutrition, and weight loss.

Our science is based on the combined efforts of physical therapists, exercise physiologists, and nutritionists who go to work for you. We have helped thousands of athletes meet or beat their goals without injury and in prime condition. Here are highlights of what just some of our athletes, backed by Phase IV and Forster Physical Therapy, have achieved throughout this year.
  • Colin Egglesfield: Forster Physical Therapy and Phase IV Client - 2015 Nautica Malibu Triathlon Celebrity-Classic Winner (First Place with a time of 16:16:59)
  • Joe Warren: Earns a shot at a title fight and his 14th Bellator win against LC Davis. That gives him the most wins in Bellator history.
  • Helen Maroulis and Adeline Gray: Win Gold Medals at the World Championships of Wrestling in Las Vegas.
  • Aaron Pico: Wins Broze Medal at the Junior Freestyle World Championships.
  • Nicole Elmes: Finisher of 2015 America's Finest Half Marathon 2015 in San Diego, CA, with a time of 1:47, she placed 389th out of 5021 total entries, placing her in the top 7% of finishers overall. She finished 113th out of 2576 female entrants, ranking her in the top 4% for all female finishers.  In her division (age 35-39) she placed 19th out of 444, placing her in the top 4% of all finishers for her age division.
  • Blossom Sato: 2014 Ocean League Player of the Year; 2014 All-CIF, Santa Monica High School Girls Volleyball - earned a scholarship to play Vollyball at Mississippi State University in 2015
  • Cindy Tilbury - 2015 Peggy Steding Female Age Group Award in Racquetball - won first place in the 55+ and second place in the 55+ Mixed Doubles divisions at the IRF World Senior Racquetball Championships.
  • Stan Kim & Andrew Eisses - Completed the 2015 AIDS Lifecycle ride from San Francisco to Los Angeles
  • Dawn Harper-Nelson: 2014 National Champion, 100m hurdles with new world-record time, 12.55 seconds; June 28, 2014 - Newly qualified for 2015 world championship team
  • Maddy Tung: 2014 Junior National Champion, 53kg folkstyle wrestling; March 30, 2014 - Now attending basic training at the US Air Force Academy in Colorado Springs, Colorado
  • Helen Maroulis - Qualified for 2015 World Championship Wrestling Team and will likely head to Rio for the 2016 Olympic Games
  • Victoria Anthony - Qualified for 2015 World Championship Wrestling Team and will likely head to Rio for the 2016 Olympic Games 
  • Rick Betts: 2014 National Champion, Mixed Doubles Racquetball; February 15, 2014
