Alert For Anthem Blue Cross Members
Big changes are coming for your Anthem Blue Cross Health Insurance coverage that will severely limit your ability to maintain your health, and the health of your family. In addition to other manufactured strategies to shirk their responsibility to pay for healthcare services, THEY WILL NO LONGER PAY FOR THE REHABILITATION OF SPORTS INJURIES, OR INJURIES THAT KEEP YOU OUT OF WORK.
Starting November 1, 2015, Anthem Blue Cross will partner with OrthoNet, a discount middle management firm, to review all requests for Physical Therapy and Occupational Therapy services and per their website:
"Unless specifically covered by a member's benefit, covered treatment goals exclude return to sport, recreational, or vocational activities."
Health insurance is supposed to make you whole again, back to the state of your health when you paid, and they accepted your premiums. That's why it's called HEALTH INSURANCE.
The way this works is that after your physician or physical therapist examines you and arrives at a treatment plan to address your problem and get you back to the things you love to do and those tasks you need to do, OrhtoNet will first require your health care professional to submit a written treatment plan before the prescribed treatment is "authorized".
The OrthoNet personnel, with no medical training and no license to practice rehabilitation medicine, will dictate the number of visits you and your family will receive, regardless of the qualified opinions of the your healthcare practitioner, who is licensed by the state of California to serve your needs, and who actually examined you and knows your medical history.
While treatment progresses, your Physical Therapist will be required to halt treatment every few visits and submit detailed progress reports and fight for more visits. You will also have to fill out weekly progress questionnaires yourself. To date, these type of discount middle management firms authorize less than half the visits required to complete a standard rehabilitation program.
These are the OrthoNet published criteria for discharging patients:
Shoulder Flexion: 144 degrees (Full Range of Motion is 180 degrees)
This level of function makes one barely able to raise the arm high enough to wash hair, and well short of reaching the top shelf of a cabinet.
Cervical Rotation: 48 degrees (Full Range of Motion is 90 degrees)
This makes it impossible for one to drive a car safely, i.e. to turn your head and look for traffic when changing lanes or for parallel parking.
Hip Flexion: 96 degrees (Full Range of Motion is 125 degrees)
When the hip joint is restricted to this degree, one cannot lift their leg to tie their shoes or transfer comfortably in and out of a car.
These criteria are more representative of an infirmed 80 year old with chronic degenerative conditions, not for healthy children and active working adults who deserve to have fully functional joints. One of the most critical aspects of maintaining healthy joints through life is maintaining full joint range of motion. It is absolutely necessary to maintain the full range of motion of your joints to preserve joint health and to prevent early joint degeneration and arthritic conditions.
Withholding care critical for your health is not a legitimate strategy to reduce medical spending. Keep in mind that Physical Therapy and Occupational Therapy services combine to be less than 2% of Anthem Blue Cross total covered expenses. Anthem Blue Cross pays less than $75 per visit in a market where the average fee for PT services is $150-175 and lasts greater than 60 minutes.
This strategy has been attempted in other states with catastrophic results for the health of millions of Blue Cross members in terms of withheld services and reimbursement denials. (see this week's feature article).
Physical Therapy is not an elective service; it is a natural and non-surgical approach to physical health, with proven functional benefits and cost saving implications. Physical Therapy has been shown to help people avoid surgery, prevent permanent disability, and reduce the need for addictive prescription painkillers,
We have to be proactive NOW to prevent this program from ever being launched and let our elected and appointed officials know that Anthem Blue Cross practices are antithetical to healthcare and expose this scheme for what it is: unethical denial of vital heath care!

OrthoNet Complaints From New York
| The new policy being introduced by Anthem Blue Cross on November 1, 2015 makes OrthoNet a middleman for Physical Therapy and sports injury rehabilitation. Not only will this change make it difficult to recieve the vists you need to get back to your full range of motion, the definition of what it means to be "rehabilitated" will be drastically altered - you will likely not be able to get the full amount of care you need to get back to your pre-injury range of motion. |
Interview from New York, NY -
More and more Americans are seeing physical therapists to help repair their ailing and injured muscles, bones, joints and ligaments. But the definition of what is "cured" is not always clear - and for thousands of New Yorkers, it may be changing. WNYC's Fred Mogul has this report.
White Plains-based OrthoNet handles physical therapy; the same way other firms manage drug, dental, or visual benefits. And it could affect you -- because OrthoNet now is the gatekeeper for three of the region's largest HMO's.
Jim Nugent, from the American Physical Therapy Association, says he probably gets five complaints about OrthoNet for every one he gets about all the rest of the country's benefits-managers. And one of the main grievances Nugent hears is that OrthoNet cuts patients off when they're "80-percent-cured."
JN: You would think you would want someone to return to the function that they had before their injury. If they're lifting boxes, and their job calls for them to lift 50-pound boxes, to go back and say, 'You can lift 40 pounds, that's okay.' A person can't go back to work; they can't do their job. But they're at 80 percent of 50, so what the heck.
FM: OrthoNet initially declined to be interviewed. Its Medical Director, Dr. Robert Refowitz, dismissed the 80-percent figure as something made up by physical therapists. Then, WNYC obtained OrthoNet documents clearly confirming the company does use 80-percent. The documents are called "Discharge Criteria." Each one covers a different condition: Torn Rotator Cuff. Knee strain. Bunion. And for all of them, it clearly states at the top: "OrthoNet's discharge criteria for range of motion, strength and function are defined as 80-percent of established normal values." The number comes up repeatedly. Confronted with its own documents, OrthoNet agreed to speak. Dr. Robert Refowitz:
RR: I can see how you might incorrectly draw that conclusion, but based on where it is actually used, it's totally appropriate in every case - range of motion, strength - it's appropriate.
FM: As an example, Refowitz opened a book called "Clinical Orthopaedic Rehabilitation" and read the description for repairing the knee's Anterior Cruciate Ligament, or ACL.
RR: They're describing ACL construction care, and they're talking about comparing one side against the other, and it says, "[if] the test of hamstring strength is 85% or better, discontinue iso-kinetic hamstring workouts."...These are standard measurements.
GL: They've misapplied that particular guideline...
FM: Gaetano Lombardo teaches at NYU and has written managed care guidelines for physical therapy. He opened the same book and found several cases telling therapists to discharge patients when they reach 100-percent strength and range of motion.
GL: Certainly the knee ligament problems - those are specific to sport populations, athletes. To take that and apply that to an individual who might be in their 50's is just inappropriate...They're taking some guidelines that are meant to be very narrowly applied, with respect to diagnosis and patient population, and they're generalizing them across diagnoses, across body parts and across populations.
FM: OrthoNet says it tailors all decisions to individual patients. Refowitz emphasizes that whatever scores patients receive for strength, pain and range of motion...what really counts is what they can do - whether that's bend over to tie their laces or reach in their back pocket to get their wallet.

The Do's and Don'ts of Running
| Just because our bodies have evolved to run, does not mean that they are equipped to handle the stress that comes with impoper running technique and damaging mechanics. Learn how to run and train smarter, not harder, with the help of Phase IV |
Economy of motion In the 30 years Robert Forster has practiced sports physical therapy in Santa Monica, Calif., he has treated world record holders, injured athletes and grandmothers recovering from hip surgery. "Our bodies have evolved to run," said Forster by telephone from London, where he was working with the U.S. Olympic Track and Field team. "Our tree-dwelling ancestors had shorter legs, longer arms and shorter feet. The body actually changed to be effective at running. But we lost our ability to co-opt that economy of motion." Forster said the current physical capacity of the average Western civilian is about 10 percent of what is possible. "Some people can run 100 miles in a day," he said. The most common running mistake he sees involves stride length, or the distance of the foot on the ground. "Everyone is over striding," he explained. "You want to land under your center of gravity, or as close to it as possible. We tend to take too few steps per minute. Less time on the ground would take care of a lot of problems." For the runner seeking greater efficiency, Forster prefers a series of running-related drills - among them arm swings that target the pendulum-like motion and knee-high drills, which he described as prancing "like a Clydesdale horse."

Don't let an old injury keep you from enjoying races, morning runs, or attaining fitness goals. In the first part of Healthy Running Step by Step, authors Robert Forster, PT and Roy M. Wallack recommend the best training based on your fitness goals, including strength training, cross training, sprints, yoga, and plenty of rest. Part two goes a step further from other books by addressing the most modern methods of treatment -- including current studies on the amount of rest required, new and controversial surgical operations and injections, the newest and most effective gear, and barefoot/minimalist running as a form of healing. If you are recovering from an injury or want to prevent becoming injured in the first place, Healthy Running Step by Step is a must-have guide.

Phase IV Athlete Testimonials |
Phase IV Scientific Health and Performance Center and Forster Physical Therapy work with people of all ages and abilities - helping them reach their full genetic potential and achieve injury-free peak performance. We strive to help everyone accomplish their health and fitness goals, regardless of how far away they may seem. We have helped thousands of athletes meet or beat their goals, and nothing is more rewarding to our team of Exercise Physiologists and Physical Therapists than seeing someone's goals realized. Here are what some of our clients have to say about their experience at Phase IV and Forster Physical Therapy:-
"I had this crazy dream of completing a marathon and quickly into my training; I realized I would need help with making this dream a reality. Today, as a marathon finisher, I cannot thank you enough for all the support in making sure my body was ready to hit the pavement for 26.2 miles and cross the finish line injury free. At mile 21, the middle of a long, curved hill, I found myself looking over and seeing runners dropping like flies with cramped quads, aching calves, and tightened hamstrings. While I may have said these words through tears, I said aloud 'Thank you Ron and thank you Forster Physical Therapy.' If it were not for your expertise, commitment to your patients, and passion for making people feel better than when they walked in, I would have been the person at mile 21 needing the medic. Thank you again for helping me make my dream a reality - completing a marathon injury-free!"
-Whitney Gaglio
-
"After a tough beginning to the cross-country season - full of injuries and health issues, Marina, my 15-year-old daughter, needed someone to put her back together. We were referred to Phase IV and after the initial evaluation, the staff provided Marina with a program that was easy to follow and fit into her high school track schedule. Marina's program included diet, strengthening, stretching, and form. Marina learned to use her heart rate as a guide for her training and ultimately ran injury-free in the 1600m, 800m, and 4X400 relay. She not only ran her personal best in track but also is leaving track healthy and looking forward to being the newly appointed cross-country team captain in the Fall."
- Lisa MacDonald - Mother of Marina Praet, 15, Phase IV Athlete and Success Story
-
"Tracy helped me by creating a nutrition and exercise program that allowed me to perform better while racing. Interestingly, I am running less that I would have on my own and getting better time and placement results. The prescribed mix of weight training exercises, intervals, recovery, and focus on heart rate has helped me to improve. I am excited to continue reaching my goals with Phase IV."
- Nicole Elmes - Finisher of the 2015 "America's Best Half Marathon" with the following finishing times and accomplishments:
Race total time: 1:47
Pace: 8:12
Overall place: 389 out of 5021 (top 7%)
Sex place: 113 out of 2576 (top 4%)
Division (35 - 39) place: 19 out of 444 (top 4%).
