Anatomy of an Overuse Injury
From Healthy Running Step by Step, by Robert Forster PT and Roy Wallack 

 
Stretching is critical to your ability to safely adapt to the physical forces that are generated during running.  Stretching maintains joint range of motion and helps the tissues heal stronger and more resilient to injury.

CHRONIC INFLAMMATION, INCOMPLETE HEALING, AND LACK OF RECOVERY


Unless you fall down and break a bone or step wrong and sprain your ankle, the vast majority of running injuries you are likely to suffer are overuse injuries. Overuse injuries occur when we outpace our body's ability to adapt to the stress of training. The micro-trauma suffered by the tissues during training, which would normally heal and make us stronger with adequate recovery instead builds into a macro- trauma with back-to-back hard days and weeks of training.
 
By the time pain arises, the typical overuse injury may be weeks or months in the making. The injured tissue most likely had been struggling to heal for quite some time, but the repetitive stress of running stalled the healing process.
 
The chain of physiological events that is put in motion to repair the damaged tissues begins with the inflammatory response. This is a characteristic reaction set in motion to first clean up the debris of the damaged cells and then begin the healing of the injured area with scar tissue.
 
These events are repeated throughout your body every time you run, and they go unnoticed if your training is progressive and designed with built-in time for recovery-and that means several recovery days every week, a recovery week every month, and some time off every year. You see, when your workouts are too frequent, or too hard, and go on for too long and without meaningful variation or enough time provided for recovery, the body struggles to stay ahead of the stress.

The Big Three Recovery Killers

Recovery is the great missing link in most training plans. The Eastern European Training guru Tudor Bompa famously said, "Recovery should be so well understood and actively enhanced that it becomes a fixed component of your training." That's because recovery-which, on the macro level, includes taking it easy the day after a hard workout or taking the day off altogether, and on the micro level includes paying more attention to your cool down, stretching, and post workout nutrition and hydration-is the crucial low-intensity time that your body needs to heal and rebuild itself so that it's better than before. Recovery is when your body actually grows stronger and more efficient. It is when the benefits of your hard training are realized.
 
"Working hard is easy - everyone knows how to work hard. But those who know how to work hard at recovery are the ones who win," says my friend and mentor, Coach Bob Kersee, the most successful track and field coach of all time. Fact: Athletes by nature know how to push it, but the ones who know when to throttle back perform the best. Therefore, to make sure you recover, it should not be left to chance. It must be programmed into your workout schedule and rigorously observed. Overtraining syndrome and lack of recovery are two sides of the same coin, and unfortunately, the rule rather than the exception. Stop them by guarding against slipping into three bad training habits:
 
1) Falling into a Rut. Beware of doing the same thing, such as running your standard 6-mile loop, every day, over and over and over. Your body adapts to the stress of a particular workout to the point where it is hardly stressful anymore. As a result, your workouts will no longer elicit a strong hormonal response; your hormones just say "ho-hum" and fade or leave.  To keep improving, you need to keep changing the stress.

Serving Up Some Physical Therapy

Crucial to Pete Sampras' continued career was his rehabilitation with Robert Forster, PT. Forster has also helped prolong the life of the bodies of athletes such as another tennis great, Maria Sharapova.


 

When tennis superstar Pete Sampras won the 2000 Wimbledon championship, one of the people he made sure to thank was his physical therapist Robert Forster, PT. It was Forster who treated the tennis great's several injuries, helping him garner the strength to make it back on the courts when many in the sport thought he should simply retire.


 
"I was treating recreational tennis players when George Fareed, the Davis Cup doctor, referred Pete Sampras to me," said Forster, who has practiced sports physical therapy in Santa Monica, California, as owner of Forster Physical Therapy Clinic for more than 20 years. "I began working with Sampras on some minor injuries. He had hurt his disc in 1999 at the U.S. Open, and there was a long rehabilitation. People doubted he could ever come back, and I made my mark in tennis because he came back and won his 13th Grand Slam [event]. I continued treating him the last years of his career."


 
After his success with Sampras, Forster was asked to work as a physical therapist for the tennis tournament in Los Angeles (currently called the Countrywide Classic), and subsequently other tennis players came calling for his private services.


 
The next was Maria Sharapova, who came to see Forster for a chronic overuse injury to her right pectoral muscle. During one match in 2005, Sharapova complained that she felt pain in the muscle when she struck her shots, and the harder she hit the more painful it became. Eventually she was forced to withdraw from the match.
