Message from the CEO

  

Phase IV physiologists and physical therapists design exercise programs that maximize health and performance gains while eliminating injury-prone movements.

Dangers in the Gym 

 

At Phase IV, most of our clients, with Internet and phone support, execute their health and fitness programs independently. They typically visit, and meet with a physical therapist and their personal exercise physiologist, for a morning of physical and metabolic analysis and personal preference evaluations. We then review the findings and the create exercise and nutrition plans to achieve their goals. Some clients execute their ongoing periodization strength and flexibility training two or three times per week at Phase IV under the watchful eye of their physiologist, or they do the workout with a physical therapist who will apply manual therapy techniques and/or massage to address problem areas along with their training.

 

Other clients, living outside the immediate area of West LA, will sometimes request a referral to a personal trainer to execute their Phase IV strength and flexibility training (even though, with the expert instruction of the workouts and photos, it is not necessary). The problem with personal training referrals is that you never know what you are going to get. The recent militarization of fitness training has created a competition among trainers to push their clients harder and harder. Accordingly, although I have been practicing sports physical therapy in Southern California for over 35 years, I am hard pressed to think of more than five personal trainers to whom I would refer my own family. 

 

Exercise is serious stuff. It can be used strengthen the body and repair problem areas, or it can create muscle imbalances that corrupt alignment and create problems. More importantly, too many injuries are created by ill-advised exercises and random workouts that are not part of a science-based fitness plan. Certification programs for personal trainers do not provide the level of understanding and knowledge of anatomy, physiology, kinesiology or patho-kinesiology (the most important science for trainers to know, because every modern day body is taxed daily with too little movement and horrible ergonomics). One also needs all of the foundational life science education that precedes these courses, in order to understand how the human body works, how and why it breaks, and how to fix it.

When did being fit become a modelling contest? Why is exercise now all about entertainment? Why are celebrities the new fitness experts? And someone please tell me how does having a million followers on Instagram make you a nutritionist? Somewhere, somehow, things have gone wrong when it comes to our health. People are confused about "how much to eat" or if "energy drinks are better than water". Really? Common sense itself should answer these questions but why are we so afraid to use it?


 
Let's take a closer look. These days it seems everyone is a Personal Trainer, Fitness Expert, Nutritionist, or Gym Instructor. You can gain these qualifications online or simply by having a good body and showing it off in a bikini on social media. Models who didn't quite make it in the fashion and beauty world have turned their attention to the fitness industry and have labelled themselves as 'experts' who want to 'inspire' people to look like them. Qualifications? Well they can fit into a size 6 pair of jeans, look good in underwear through a bathroom mirror, and buy millions of followers on social media.


 
If you are a chef, cook, or waiter at a cafe and handle food then you too can have a qualification as a Nutritionist/Dietician. There is no regulation against it and this is where the problem begins. These days you can dish out any exercise or food advice on social media and magazines which in my opinion is grossly irresponsible. Sure you may not get adequate insurance but even that process has become somewhat of a joke. The insurance companies are dancing the tango with the qualification providers and we now have loop holes bigger than the ozone layer.

Lactic Acid: Friend or Foe, and Why Test For It?

 

There has been a lot of confusion about lactic acid and what it means to the performance athlete and their training.

 

The confusion seems to be born from two facts. First, science certainly does not have all the answers when it comes to exercise training and the limiting factors in performance. Example: For years scientists thought lactic acid caused fatigue. Now it's understood that in itself it does not cause fatigue but at the point where it collects in the muscle cell, fatigue is quick to follow.

 

Secondly, what is known is often taken out of scientific context and expected to be a stand-alone truth. Example: Lactic acid is a terrible thing because training programs are designed to reduce its production. Lactic acid is a natural by-product of anaerobic exercise and, no, it's not all bad - but we still want to limit its production at certain times in the training cycle.

 

The truth in these statements is only evident with a complete understanding of muscle physiology and the goals of endurance training programs. First, a review of exercise physiology is helpful. In the continuum of exercise intensities there are two definite domains. At the lower intensities the majority of muscle energy is generated by aerobic metabolism of FATS as a food substrate. To metabolize fats, which are virtually unlimited even in the thinnest of athletes, the muscle cells need an adequate supply of oxygen. As exercise intensities increase the muscles begin to work so hard that they outpace the body's ability to deliver oxygen. Metabolism then shifts into the anaerobic domain. Here the muscle cell must metabolize CARBOHYDRATES to produce muscle energy with the by-product of lactic acid.

 

At first this is no problem as the lactic acid is produced at a rate which can be used by the aerobic muscle fibers as a fuel, buffered by the chemicals released in the cell or cleared from the cell and taken away by the blood vessels. Prolonged exercise at this intensity, however, creates problems as it consumes all available carbohydrates and the body cannot re-synthesize enough to maintain this exercise intensity (the bonk).

 

At this point the athlete is forced to slow down or stop exercise all together as the finite amount of carbohydrate (glycogen) stored in the muscles and liver is totally exhausted.

This is rare. To have an athlete with his mind set, an athlete who does every single thing you tell him to do to be sure he is 100% prepared, and with a great strategic technique coach, who gets the science and applies it and to gets him here injury free and peaked so we will be able to watch his athletic and competitive skills shine, believe me it is rare."
