It's Crunch Time for Injured L.A. Marathoners
It's marathon season in Los Angeles. We are just four weeks away for the 30th running of the Asics Los Angeles Marathon. As the Exclusive Physical Therapy Provider of the L.A. Marathon, the staff from Forster Physical Therapy and Phase IV have been found doing community service with the Road Runners L.A. Marathon training group nearly every weekend for the last three months. Performing complementary injury assessments; helping perfect running technique; teaching advanced recovery techniques through stretching, foam rolling, and icing; we have been busy trying to keep runners healthy. We are busier than we need to be.
For over three decades, we have tended to runners' needs, and while the advances in endurance training science have been significant, unfortunately the training acumen of the average marathoner has progressed very little over that period. These scientific advances are evident in elite runners who continue to break records, however this information is not getting to the masses who need it most. They still suffer needlessly. Our work with the L.A. Marathon over the last eight years has been focused on improving the average runner's understanding of training science and injury prevention.
These experiences, and our work with Olympic runners who find themselves injured right before a major competition like the Olympic Games, have led us to develop the skills to quickly resolve injuries while maintaining hard-earned competitive fitness.
This week's feature article covers the essential areas of focus that all injured athletes must accomplish for a hasty and permanent resolution of their symptoms. This is a must-read for anyone running the L.A. Marathon on March 15, or for anyone suffering a sports injury. Pass it along to the runners in your life.

Last-Minute Focus for the L.A. Marathon
 | Phase IV physical therapists have helped runners achieve their goals for over 35 years. As the Official Physical Therapy Provider of the L.A. The statistics speak for themselves: 80% of runners will suffer an injury every year. If you or someone you know is inured, and the race is eminent, you have to act quickly and decisively to resolve the injury and find the cause while you maintain your hard-earned fitness. This is our specialty and the reason that Forster Physical Therapy has been the "go to" spot for injured athletes for over 35 years.
Runners who suffer an injury right before their goal race need to focus on four things:
1) Attaining the correct diagnosis immediately
You can't fix an injury until the damaged and pain-provoking structure is identified precisely. It should become the focus of treatment techniques and protective measures designed to prevent further irritation and damage. Various clinical testing and skillful and thorough palpation techniques are the tools of success when it comes to finding painful structures. Only then can the right treatment techniques can be applied.
2) Detecting the mechanical cause of the injury
My physical therapy mentor, John Papalia, PT, is fond of saying, "The human body is famous for hurting where it ain't hurt." Meaning, the pain associated with an injury is not always experienced at the injury site, but instead can be referred to various areas remote to the injury. Additionally, the most common causes of injury are mechanical problems near or distant to the pain. Any mechanical asymmetry in what's known as the "kinetic chain," which includes the joints of the ankle, knee, hip, pelvis, and spine, can cause an injury far from the dysfunctional area. Detecting these mechanical deficits is absolutely essential as the first step towards resolution. This is our specialty.
3) Getting the right treatment
Many clinicians who claim to do sports medicine will sell athletes on laying on a table and receiving passive treatments from "fancy black boxing," which is supposed to work to miraculously heal injured tissues. Be wary. Although we use fancy therapeutic modalities like ultrasound, electrical stimulation, and icing, these do nothing more than help suppress the symptoms while we actively rehabilitate the injury and fix the cause. The operative word being "actively." We make every injured athlete an active participant of their rehabilitation, and never a just passive recipient of care. The body is a mechanical structure and therefore requires mechanical treatment. As such, stretching, strengthening, cross fiber massage, and ergonomics adjustments are the critical active therapeutic measures that will resolve your injury.

Proper Hydration Yields Better Performance
Summer is here and the heat is getting turned up in Southern California. For cyclists and runners this means dehydration and more serious conditions like heat stroke and hyponatremia. Hyponatremia is a decreased concentration of sodium in the blood, which can lead to serious physiological conditions. Avoid hyponatremia or water intoxication (a very dangerous condition) by taking in fluids with electrolytes during hyper hydration efforts.
It is also important to take extra care to maintain healthy fluid and electrolyte levels to prevent heat stroke and muscle cramping.
Beyond Thirst
Thirst is a good signal that it's time to get a drink. But if you wait till you're thirsty while exercising, you've waited too long. Thirst is the first sign of dehydration.
Why Do I Need Electrolytes?
Electrolytes help maintain proper fluid balance as well as muscle and nerve functioning. The most commonly measured electrolytes are sodium, potassium, chloride, and bicarbonate. We lose electrolytes when we sweat. Also, too much plain water will flush these out of your system. Maintaining a healthy balance of electrolytes in the body is critical. Using sports products like electrolyte tablets in your water bottle will ensure proper electrolyte replacement. A pound of sweat contains between 400-700 mg. of sodium. As always, how much sodium you lose is individual to you. As you become acclimated to the heat, your body will become more sweat-efficient and you will lose less sodium. What is the danger? As you become sodium depleted (or hyponatremic), you will not absorb fluids as efficiently in the stomach and the result will be muscle cramps and fatigue.

When ADVANCE's Brian Ferrie compiled an account of how skiers take advantage of physical therapy treatment, he turned to Phase IV founder and CEO, Robert Forster, PT.
"Phase IV is sports training," [Forster] explained to ADVANCE. "Many of our patients will finish their rehab there, and some will continue on their fitness programs. It's like a personal training gym staffed by physical therapists and exercise physiologists."
