Injury Stats and Observations From This Year's Los Angeles Marathon

 

Healthy Running Step by Step, by Robert Forster, PT, and Roy M. Wallack, is due out this Fall and is a recommended read for next year's marathoners.

The last two weeks have been busy at Forster Physical Therapy, with LA Marathon runners coming in for last-minute injury resolution and minor tune-ups. Over the last several months, my staff and I have performed over 150 free injury assessments for runners competing in this year's race in our role as the exclusive Physical Therapy provider of the Los Angeles Marathon. We have seen runners of all ages and sizes desperate for a chance to get to their goal of finishing this year's race.


The three most common injuries we diagnosed in this self-motivated and driven patient population were illiotibial band syndrome, achilles tendonitis, and muscle-tendon injuries of the shin, also known as shin splints. We were fortunate to offer them a chance to line up at the starting line feeling better after physical therapy intervention. Undoubtedly, a number of athletes injured during the the race will schedule in the coming week and if history is any indication, most of these runners could have avoided their injury with a more science-based approach to their training. Our goal for all of our clients is to prevent unnecessary injury and bounce back stronger and smarter for their next fitness goal.


Observations


Motor over Mechanics:
Apparently, when it comes to fast finishes in marathon racing it is motor over mechanics. Watching for good running form at mile 24 we saw many more runners who would go on to achieve a respectable sub-three hour finish than we saw mechanically sound runners -- meaning the vast majority of fast finishers had the genetic gift and aerobic foundation to maintain better than a 7-minute/mile pace for 26.2 miles in spite of less than optimal running mechanics. Think of how much faster they could have finished if they ran with proper form. Similarly, for the post-three hour finishers, the genetic and aerobic performance gap was widened further with bad form.

Barefoot Running R.I.P.:

Fortunately, bare foot running is apparently another failed fad as I only saw one barefoot runner come past the 24 mile mark in the first four hours of the race. The reported injuries have apparently outweighed the proposed benefits of this ill-advised trend that has taken runners down the wrong path. Recent research has documented the plethora of foot and leg injuries that occur when tender-footed runners discard their shoes in search of mechanical nirvana. It was obvious to us from the start that good running form does not require a shoeless approach. With a few strength exercises and some coaching, any runner can make their running style more efficient and less likely to create injury.

Race Round-Up From the 2014 Asics LA Marathon

The 29th running of the Los Angeles Marathon was success and a historic event on the 30-year anniversary of the '84 Olympic Games that inspired this now world-class race. Over 25,000 runners covered the 26.2 mile distance under stressfully warm conditions. The elite men's winner was Ethiopian Gebo Burka, with a time of 2:10:37, four minutes off the 2:06 course record set by countryman Markos Geneti in 2011.

 
The first female finisher was another Epiopian, Amane Gobena, who ran a 2:27:37 and took home a $75,000 bounty -- $25,000 for the first female finisher and a $50,000 bonus for winning the gender challenge by coming in first overall after she and the elite women's field started with a 17:41 head start over the men. Burka got within 41 seconds of Gobena, but ultimately couldn't close the gap in what became a very exciting and costly finish for him. 

The unusually warm conditions prevented the defending champion, Erick Mose from Kenya, from repeating his win with a predicted time of 2:06; he instead instead finished in 2:12:56 for third place. Of the top six women's and men's finishers, there were three Ethiopians, two Kenyans and one American, Lauren Kleppin, who was impressive in her third-place finish among this world class field in her very first marathon.
 
Here is the play by play action starting at the half way point as reported by the Los Angeles Times:
 
Habtamu Asefa of Ethiopia leads a group of five runners who appear to be pulling away at the halfway mark in the men's race of the 29th Asics L.A. Marathon.

Asefa passed the 13.1-mile mark in 1 hour 5 minutes 57 seconds.

Ethiopia's Tigist Tufa and Amane Gobena broke away from the lead women's pack at the 18-mile mark in the 29th Asics L.A. Marathon.

Tufa clocked 1 hour 45 minutes 34 seconds through 19 miles.

Lani Rutto of Kenya, Gebo Burka of Ethiopia and Dawit Wolde of Ethiopia are leading the men's race in the 19th mile

Defending champion Erick Mose fell off the pace. Kenyan Julius Keter, who finished second last year, is in the lead pack

With 6.2 miles left in the 29th annual Asics L.A. Marathon, two women and two men are pulling away in their respective races.

Tigist Tufa of Ethiopia clocked 1 hour 51 minutes 9 seconds for a narrow lead over Amane Gobena in the women's race.

Lani Rutto of Kenya clocked 1:39:53 for a slight lead over Ethiopia's Gebo Burka in the men's race.

Dangers in the Gym!

Every day we see research that shows exercise is the best medicine to keep your body working well and for the prevention and treatment of many of the lifestyle diseases that plague our population. Exercise is powerful medicine and, like with all medicine, proper use is critical to get the best results and avoid injury.

 

For over 30 years I have traveled the world and witnessed all types of exercise programs. I evaluate every exercise move in a risk-benefit analysis. If the potential of injury is greater than the benefit, or if a better result can be attained with a safer move, the exercise is not prescribed. 

 

Performance and Safety Tweak: Abdominal Isometric

 

While the traditional crunch (L) places strain on the neck and trains the abdominals in a way that does not translate to sport or daily life, the isometric partial sit-up eliminates such strains and trains the abdominal musculature for the static nature of its workload.

 

Problem: In everyday movements and postures, the abdominal muscles stabilize the core and protect the spine via isometric contraction.  So why do most people train the abdominal muscles in repetitive movements (crunches) that train the muscle isotonically (through a movement pattern) and subject the neck to stressful jerking motions?

 
Solution: To train the abdominal muscles more like they function in real life and prevent repetitive stress on the neck, lay flat on your back, knees bent and feet on flat on the floor. While keeping your head back and aligned with your shoulders and face pointing towards the ceiling, perform a curl-up motion that stops when the shoulder blades leave the ground. Hold this isometric contraction for a five count and return to the starting position. Repeat five to ten repetitions for a five count hold, and then either increase the hold time or increase the reps as you progress in strength. 
 

Always stretch BEFORE and AFTER each workout to stay injury-free.
