The History of Sports Physical Therapy for Runners: Part I

 

Excerpt from Healthy Running, Step by Step, the new book for runners by Robert Forster, PT, and Roy Wallach. Coming this Summer from Fairwinds Press.

 

In 1978, when I first began treating runners, they were seen as an enigmatic group of loners who spent hours of solitude slogging through longer and longer workouts. With increased awareness of the health benefits derived from regular exercise, a segment of the U.S. population was motivated to get active and they laced up whatever athletic shoes they had and hit the roads. After all, running was easy, required little investment, and everyone ran as a kid so they "know" how to do it.

 

The exhilaration that comes with a repetitive cyclic motion like running and from the wind in one's face hadn't yet been identified as "the runners high" or tied to the release of morphine-like chemicals in the brain, called endorphins. But runners knew it made them feel good and before long there were millions of runners hooked.

Everything You Need to Know About the 2014 Asics LA Marathon

 

The 2014 LA Marathon's 26.219-mile course runs from Dodger Stadium to Santa Monica, CA. Course map courtesy LA Marathon.

Since its inception in 1986, the Los Angeles Marathon -- launched to build off the success of the 1984 Summer Olympics -- the race has highlighted Downtown LA with a route running a loop around the city center to and from the LA Memorial Coliseum.

 

This year, however, marks the fifth year of the marathon's new route from Dodger Stadium to Santa Monica. With the original Olympic route, which saw a then-Olympic Record winning time of 2:09:21 by Portugal's Carlos Lopes (with Alberto Salazar, who became well-known outside running circles when Lance Armstrong recruited him to coach his attempted foray into triathlons, finishing 15th 2:14:19) and its marathon iteration providing hill-suppressed finishing times that lagged behind those of other major marathons, organizers found a new way to highlight not only Downtown, but also surrounding areas along with Hollywood and Beverly Hills with the current, 26.219-mile route.

 

Through last year's race, notable participants have included Shia LeBeouf, Alanis Morissette, and Gordon Ramsay, and of the 19,534 entrants in 2013, Nick Arciniaga (2:17:05) and Deena Kastor (2:32:39) were the fastest Americans -- Kastor, aged 40, was not only last year's top female masters racer, but the top masters racer overall, beating Abderrahmane Djemadi by a full five minutes. Age, then, really is little obstacle to athletic performance if the training is right.

 

For racers, fans, and spectators, the NutriBullet Health and Fitness Expo, held in the West Hall of the LA Convention Center, will be the main pre-race draw. On Friday and Saturday from 10 am 7 pm, and 9 am to 5 pm, respectively, runners will arrive to grab race packets, and both runners and fans can visit just over 140 sponsor exhibitions along with a day-long speaker series on the main stage with presentations on nutrition and training; Phase IV's Robert Forster, PT, will speak on implementing scientific training principles Saturday morning at 9:30 a.m.

Every day we see research that shows exercise is the best medicine to keep your body working well and for the prevention and treatment of many of the lifestyle diseases that plague our population. Exercise is powerful medicine and, like with all medicine, proper use is critical to get the best results and avoid injury.

 

For over 30 years I have traveled the world and witnessed all types of exercise programs. I evaluate every exercise move in a risk-benefit analysis. If the potential of injury is greater than the benefit, or if a better result can be attained with a safer move, the exercise is not prescribed. 

 

Faulty Running Mechanics: Over-striding

 

If someone were to take up golf, they would most likely consider taking golf lessons. If they join a tennis league, there would undoubtedly be instructional resources to learn the mechanics of the serve. However, since we all have been running around since the first years of our lives, we mistakenly think that we don't need to understand or learn the mechanics of efficient, injury-free running. The principles of proper running mechanics need to be understood and applied to reach your highest potential as an efficient and injury-free runner.

 

The human body is built to be an efficient running machine. Evolution provided the adaptations that allow us to move our bodies economically over long distances in search of food. The problem is that our sedentary lifestyle denies us the strength and flexibility to exploit the built-in adaptations that make us efficient runners; we have to re-learn how to use the evolutionary gifts. 

 

Problem: Over-striding is defined as taking too long of a stride - therefore, the foot lands too far in front of our body. Run gait is comprised of two distinct phases: the stance phase, and the swing phase. The stance phase is the time during which your foot is on the ground and the leg is bearing the forces of body weight. The swing phase of the running gait cycle is when your leg leaves the ground to recover from a position behind your body, to land on the ground in front of you to propel you forward again. Running differs from walking in that in the walking gait there is always at least one foot on the ground, and a period called double stance, where both feet are on the ground at the same time. This, of course, limits the amount of force coming down with each foot strike, as some of the body's weight is always supported by one leg.

 

Conversely, there is a time in the run gait cycle called the float stage, during which neither leg is on the ground and we are literally flying through the air. Accordingly, in running the forces absorbed by the foot and leg at each foot strike approach dangerously high levels, up to four times your body weight. Efficiency and speed in running is determined by where the foot strikes the ground in relation to the center of mass of your body. The further in front of the body that the foot hits the ground, the more inefficient we are at propelling our body mass forward. Wasted energy translates to slower speeds. Additionally, when you over-stride, the joints of your foot and leg operate from a disadvantageous position, working too hard to propel you forward and subjecting the tissues and joints to excessive and unnatural forces - which creates injuries.
