This week, our feature article is the first in a series focusing on the management of running injuries. The truth is that anyone suffering from an injury, whether they run or not, will gain valuable insight from this series, as it will delve into the causes and remedies of ailments common to many sports.

 

Forster Physical Therapy has been designated the Exclusive Physical Therapy Provider of the Asics Los Angeles Marathon for good reason: more than half way through our fourth decade of treating running injuries and helping runners train for injury-free, peak performance, this is a role that puts us right where we shine. Our approach to runners' health is anchored in the robust body of science that has been assembled over these decades, and the location of our practice -- at the center of one of the biggest running meccas in the world -- has provided the ultimate proving ground for our staff to hone their skills.

Just like the LA Marathon itself, our practice was born in the era of the 1984 Olympics in Los Angeles, where we helped seven American track and field athletes win 10 Olympic medals. That began a 30-year tradition that continues today. Our athletes have won at least one track and field gold medal in each of the last eight Olympic Games, 44 medals in all.

 

After seven years serving the members of the LA Marathon Roadrunners Training Program, we are proud to now serve ALL runners competing in the LA Marathon. To celebrate our designation, we are extending free injury assessments to all registered participants of the 2014 LA Marathon. Pass on our offer to anyone running the race this year.

Anatomy and Function of the Achilles Tendon

The Achilles tendon is one of the most robust tendons in the body. Three relatively large and extremely strong muscles in the calf all attach to the back of heel bone (calcaneus) via the Achilles tendon. This particular tendon, like many others in the body, is encased in a sheath called a paratenon that produces a lubricating fluid designed to prevent friction between the tendon, its sheath, and other structures the tendon contacts.

 

The forces generated through this tendon during running are some of the largest anywhere in the body. When a muscle contracts and shortens, it is called a "concentric" contraction; when it lengthens in a controlled release of its contraction, it is called an "eccentric" contraction. During a toe raise exercise, the calf muscles contract concentrically and, via the Achilles, lift the body weight on to the toes. When you consider that the average, untrained man can do a standing toe raise with somewhere around 200 pounds on his shoulders, and a well-trained man can lift over 400 pounds with his calves, you can begin to appreciate the magnitude of forces transmitted through the Achilles tendon during activities such as uphill running, or when jumping across puddles and over parking bumpers.

 

During flat, steady-state running, the calf muscles perform an eccentric, or lengthening contraction, to slow the forward progression of your body weight as the shin bone moves over the foot in mid-stance.

 

Pain Presentation

 

There are three different conditions of Achilles tendon dysfunction, and their pain presentations have similarities -- but with careful observation and palpation, it is possible to provide a more exact diagnosis. All Achilles tendon injuries hold the risk of complete tendon rupture. In fact, the Achilles tendon sometimes suffers spontaneous rupture with no history of previous injury or pain. For these reasons it is imperative to stretch the Achilles tendon daily and keep the calf muscles strong.

 

Achilles Tendonitis

 

The Achilles tendon, like all tendons, is comprised of collagen fibers that are constantly reacting to the stress of the forces created during activities of daily life and exercise. In normal use, the body has little problem keeping up by repairing fibers damaged under the loads of walking, running, and jumping. Achilles tendonitis occurs when the stress of these activities outpaces the body's ability to repair micro damage to the fibers. Chronic inflammation results and pain is soon to follow.

Every day we see research that shows exercise is the best medicine to keep your body working well and for the prevention and treatment of many of the lifestyle diseases that plague our population. Exercise is powerful medicine and, like with all medicine, proper use is critical to get the best results and avoid injury.

 

For over 30 years I have traveled the world and witnessed all types of exercise programs. I evaluate every exercise move in a risk-benefit analysis. If the potential of injury is greater than the benefit, or if a better result can be attained with a safer move, the exercise is not prescribed. 

 

The Seated Torso Twist

 

Problem: This machine is designed to shape the midsection by working the abdominal oblique muscles. This machine is popular with gym-goers looking to eliminate love handles by "spot reducing" fat off their waists. The first problem is that "spot reducing" is a misnomer because fat comes from the body in an overall uniform manner and working the local area where our bodies hold fat does nothing for reducing it. More importantly the sitting posture is a poor choice for exercising any part of the body because the spine is in a very vulnerable to injury in this position. When exercising in sitting the lumbar disc pressure is elevated to dangerous levels running the risk of bulges and disc herniation.

 

Solution: Work the abdominal obliques lying flat on the back and doing a partial isometric sit-up with rotation. In the supine "flat back" position gravity is not bearing down and exposing the lumbar disc to excessive pressure. In standing, the obliques can be worked safely with wood chops resisted by cables or rubber tubing. Always stretch before and after each session in the gym and use the "press up" exercise to protect the lumbar discs from injury. 

 

Always stretch BEFORE and AFTER each workout to stay injury-free.
