Beginning January 2014 the professional staff at Forster Physical Therapy has been granted the right to evaluate and treat patients without having to first see a physician to make a diagnosis and provide a referral to PT services. After decades of advocacy for this new law, health care experts have succeeded in their fight against the California Chapter of the American Medical Association to allow the general public direct access to Physical Therapy services for patients with acute musculoskeletal injuries and chronic joint and tendon conditions. "The proven cost savings of direct access for Physical Therapists granted in other states could no longer be stifled by the turf protection practices of the California medical profession" reports John Papalia PT, a retired clinical instructor with over 40 years of experience in the field. "With health cost savings dominating the conversation across the nation and across the political spectrum, and with the education standards of today's Physical Therapists, this only makes sense."

The practice of seeing an orthopedic surgeon for obvious non-surgical injuries such as a sprained ankle, tennis elbow, rotator cuff strain and low back pain makes no sense financially or medically. Patients often complain about the cost and wasted time of an MD office visit, with unnecessary x-rays and MRI's and a two minute exam, just to get referred to Physical Therapy. At the same time, public opinion has veered away from medications, injections and avoidable surgeries in favor of the more natural approach of rehabilitation exercises, ergonomic instruction and education that characterizes the standard Physical Therapy approach. Physical Therapy Works to Avoid Surgery

Mar. 21, 2013 - A New England Journal of Medicine (NEJM) study showing that physical therapy is just as effective as surgery in patients with meniscal tears and arthritis of the knee should encourage many health care providers to reconsider their practices in the management of this common injury, according to the American Physical Therapy Association (APTA).
The study, published March 19, showed no significant differences in functional improvement after 6 months between patients who underwent surgery with postoperative physical therapy and those who received standardized physical therapy alone.
"This study demonstrates what physical therapists have long known," explained APTA President Paul A. Rockar Jr, PT, DPT, MS. "Surgery may not always be the best first course of action. A physical therapist, in many cases, can help patients avoid the often unnecessary risks and expenses of surgery. This study should help change practice in the management of symptomatic meniscal tears in patients with knee osteoarthritis."

Every day we see research that shows exercise is the best medicine to keep your body working well and for the prevention and treatment of many of the lifestyle diseases that plague our population. Exercise is powerful medicine and, like with all medicine, proper use is critical to get the best results and avoid injury.
For over 30 years I have traveled the world and witnessed all types of exercise programs. I evaluate every exercise move in a risk-benefit analysis. If the potential of injury is greater than the benefit, or if a better result can be attained with a safer move, the exercise is not prescribed.
Safer, Effective Alternatives to Three Common Gym Exercises
1. Knee Extension Machine (works quads) The movement on this machine is unnatural and holds great potential for injury. Moving the knee against resistance from such a flexed position to the fully straight position subjects the back of the knee cap and the end of the thigh bone to great compressive forces that will eventually wear through the cartilage and create a hole that will never heal. The result is osteoarthritis. Solution: Leg presses and squats work the quadriceps in a more natural movement because the forces are delivered through the foot, while both the thigh and shin bones are moving at the same time -- and this subjects the knee cap to much less compressive force. 2. Behind the Neck Lat Pull-Down (works lats and shoulder blade stabilizers) Doing pull downs behind the neck subjects the shoulders and neck to great strain, and does not necessarily work the muscles of the back any better than safer alternatives. Solution: Lean back instead and pull the bar down to your chest to work the same muscles and avoid neck and shoulder strains. 3. Full Range of Motion Bench Press (works pecs, shoulders, and triceps) Pushing the barbell off of the chest positions the shoulder in a vulnerable position, subjecting the front of the joint to excessive forces. The biceps tendon is stretched excessively and can damage a structure deep inside the joint -- called the labrum -- which often can only be repaired with surgery. Solution: Only lower the barbell to a position approximately five inches off of your chest, and then raise to the starting position. In other words, don't let your elbows pass the plane of your body when lowering the weight.
Always stretch BEFORE and AFTER each workout to stay injury-free.
