Progress in Body Maintenance

For me, it's been a professional preoccupation to get the public to realize the human body requires maintenance just like our teeth. It occurred to me decades ago that the average person spends more time taking care of their teeth than they do the rest of their body. The American Dental Association has done a much better job than the American Physical Therapy Association in getting people to think about health maintenance and disease prevention. Most of us brush multiple times per day, floss regularly and get in for cleanings to keep our teeth healthy. But few stretch daily or do basic strength work and nearly no one ever sees a physical medicine clinician to learn where they need to focus preventative care measures for their bodies.

Our mostly sedentary lifestyle leaves our bodies weak and tight creating dysfunctional movement patterns that injure soft tissue structures and damage joints. Even for those who exercise daily, the body remains motionless for the majority of their day and inactivity has been shown to cause cellular changes throughout the structural tissues of the body.

Recently, I have realized that while my campaign to get adult people to do basic body maintenance with simple stretching and strengthening exercises has been difficult until they get injured, the message has been received and taken seriously by the younger generation. First I was struck by how much our young Olympic athletes do for themselves to stay healthy. When I first began working with Olympians in the 1980's the number of gold medals won by individual athletes were directly proportional to the number of times a week I saw them in our clinic for health maintenance. Jackie Joyner Kersee came in for preventative care the most and she won five Olympic medals, FloJo was a close second and she earned four Olympic medals. Through stretching, massage, icing and other  recovery modalities we were able to prevent most injuries and when problems did occur I would often be able to detect and treat them before they became full blown injuries. The Olympians we work with today do much of their own body maintenance, working out tight muscles and scar tissue with foam rollers, softballs and home ice baths.

Most excitedly, these self-help strategies have not only been adopted by the professional athletes in their quest to stay injury free, but now I see the foam roller has become a regular tool for many in the gym. When I recently witnessed a young teenage swimmer going to practice with his orange foam roller attached to his backpack I began to think there is hope for the younger generation to save their joints and achieve their highest athletic potential, injury free.

ESPN Sports Science Show Profiles
Phase IV Fitness Testing

ESPN's Emmy Award-winning TV series, hosted by John Brenkus, uncovers sports' biggest myths and mysteries by using cutting-edge technology to measure momentum, friction, human physiology and the laws of gravity. For a recent episode, featuring Indy Car driver Will Power, Sports Science contacted Phase IV Scientific Health & Performance Center about airing a VO2 test with Will Power to prove, among other things, that even a race car driver must test and train aerobically to withstand and excel through the pressures and physical challenges of an athlete. Click Here to watch the video

Over the course of a race, of up to and over three hours, it is imperative for the driver to take in and utilize as much oxygen as possible to continue to be aerobic throughout the entire race. When we are in an Aerobic state we are utilizing oxygen to burn more fats as fuel than carbs. Fats are a much more sustainable source of fuel as our body will have about 80,000 calories of fat at any given time and only about 2,000 calories of carbohydrates. So if Will was not in an Aerobic state during his races but instead he was Anaerobic he would burn through those carb stores very quickly and while the car may have fuel his body would have run out, putting his life on the line while driving in excess of 230 mph.

Taking the VO2 or Volume of Oxygen test allowed us to show Will his aerobic capabilities as well as how many calories he burns at different heart rate zones and what fuel source he is using in those zones. At Phase IV we use this test to provide our athletes as well as weight management clients the proper heart rate zones, based on their specific DNA and physiology, in which they must train in to increase their oxygen utilization, fat burning capabilities and overall performance.

After receiving the results of the VO2 test the athlete will use a heart rate monitor to ensure they are training in the proper zones. After just a few weeks the results can already be seen when metabolically training because you can go longer, faster and burn more fat while at the same heart rate. Transforming your physiology in this way will increase performance and the results that are manifested not only during the aerobic stage but as you go into the anaerobic stage as well, allowing for greater performance in areas such as mental focus as well as coordination. Watch this short video from ESPN's Sports Science to see how Will Power does.

Dangers in the Gym!

Everyday we see research that shows exercise is the best medicine to prevent and treat many of the lifestyle diseases that plague our population. However, many people are not sure what to do in the gym and they are rightfully concerned about getting injured. Exercise is powerful medicine but proper dosage and use is critical to avoid problems and assure success.

At Phase IV we evaluate every exercise in a risk-benefit analysis. If the potential of injury is greater than the benefit, or if the same result can be attained with a safer move, the exercise is not prescribed. In this new weekly column we will address one dangerous exercise each week.

Women on Weights

Problem: Many women forego weight training for fear of getting "bulky". This is most unfortunate since women have the biggest upside in terms of the benefits of weight training. First, women of all ages need to be concerned about the disabling effects of bone loss later in life. Osteoporosis is a loss of bone density and is an insidious problem that all women face. Two out of four women will suffer an osteoporotic bone fracture in their lifetime and this percentage will be getting higher as experts now say that 40 per cent of American women alive today will live to be over 100 years old. Women today are well served by the famous George Burns quote: "If I knew I was going to live this long, I would have taken better care of myself." It is never too early to put more bone in the bank as bone density peaks for the average woman at age 25 and it's all down hill from there without some form of resistance training to stimulate bones to drink in more calcium.

Secondly, it is a scientific fact that women have weaker upper bodies and more flexible joints than men and thus suffer more upper extremity and spine injuries. Strength training improves the stability and function of the joints of the wrist, elbow, shoulder, thoracic spine and neck helping to avoid these injuries in an increasingly active female population.

Lastly, for female athletes, upper body strength training can provide the competitive edge over opponents who do not weight train.
