Experienced Physical Therapists can predict injuries common to specific sports, popular fitness trends and even some occupations. Swimmers, tennis players and other overhead athletes are prone to rotator cuff injuries of the shoulder; disciples of Crossfit and P90X often present in our clinic with overuse injuries of the spine and lower leg, while lawyers and other "desk jockeys" commonly have neck issues related to disc protrusions and arthritis. When given the task to create fitness programs for athletes and the general public, who better than Physical Therapists to build in protective exercises to avoid the injuries common to their lifestyle, sport or occupation? The Benefits of an Active Lifestyle and Long,
Low Intensity Exercise

Our bodies were made for movement. Inactivity and sedentary lifestyles have been blamed for many of the common diseases that plague our population including; obesity, diabetes, heart and vascular disease and even some cancers. Apparently, even the most disciplined among us who exercise vigorously for one hour a day are susceptible to the dangers of being largely inactive the rest of the day.
New research suggests there are significant health benefits linked to a more active lifestyle and long, slow workouts than with short, intense workouts. Longer, low intensity aerobic exercise (including walking, cycling, swimming, etc.) force your body to burn fats better and improves the way your body processes sugar as well. For athletes and the general public, aerobic activities reprogram your body to use fat as its preferential fuel and will enable us to work harder for longer periods of time with less fatigue. Dangers in the Gym!
|
Everyday we see research that shows exercise is the best medicine to prevent and treat many of the lifestyle diseases that plague our population. However, many people are not sure what to do in the gym and they are rightfully concerned about getting injured. Exercise is powerful medicine but proper dosage and use is critical to avoid problems and assure success.
At Phase IV we evaluate every exercise in a risk-benefit analysis. If the potential of injury is greater than the benefit, or if the same result can be attained with a safer move, the exercise is not prescribed. In this new weekly column we will address one dangerous exercise each week.
DIPS
As a result of the evolutionary process our bodies all share common areas of vulnerability. These weaknesses relate to the limitations of our body tissues to withstand the forces they are subjected to in our daily activities, fitness movements and sport. As Physical Therapists we quickly learn that the joints of the spine, knee and shoulder are the most commonly injured and therefore require us to protect them judiciously in all our activities including exercise. For the shoulder, the common mechanism of injury is that the structures in front of the joint are insufficient to withstand the repetitive forces that are created in our activities. The ligaments and joint capsule in front become too lax while the structures in back of the shoulder become too tight. This common condition displaces the head of the arm bone (humerus) to a forward position in the joint and causes it to smack into the roof of the shoulder (the acromion) when we use our arms in overhead positions. This leads to the all too common rotator cuff impingement syndrome and arthritic conditions of the shoulder. All exercise that puts unnatural stretching forces on the front of the shoulder must be avoided to protect us against injury.
Problem: Dips are performed either on a dip bar apparatus with the legs hanging below or off a flat bench with our arms behind us and feet on the floor in front of us. When we bend the elbows to lower our body, the top of the upper arm bone (humeral head) is forced forward subjecting the ligaments and joint capsule protecting the front of the joint to become stretched out. Over time this creates laxity and a poorly aligned joint susceptible to dislocations, impingement syndromes and damages to the bones themselves.

KENT MOODY, MPT, DIRECTOR OF CLINICAL QUALITY ASSURANCE
Kent received his Master of Science degree in Physical Therapy from the University of Southern California in 1977 and his B.S. in Health Education from the University of Oregon in 1975. Kent joined Robert Forster in October 2000 as a Staff Physical Therapist and Head of Clinical Quality Assurance after having worked at HealthSouth Kerlan-Jobe Orthopedic Clinic since 1993. He has also worked at Southern California Center for Sports Medicine in Long Beach, S.C.O.R., Inc. at Cigna Health plans in Los Angeles in cardiac rehabilitation, and at Daniel Freeman Hospital in cardiac rehabilitation. Kent specializes in shoulder, knee, and back injuries and surgeries and is extremely proficient in cervical, elbow, and foot/ankle injuries as well. With over 35 years as a Physical Therapist, his knowledge and expertise enables him to treat all patients with a very high level of success. Additionally, the area of ergonomics and its relation to prevention and treatment of injuries is a special interest. Kent has been an avid runner since 1967 and has participated in numerous 10k runs and half marathons. He is also involved in swimming, surfing, skiing, backpacking, and camping.
