CHANGE TO SERVICE DELIVERY AGE FOR ADOLESCENTS |
|

New rules are now in place for the Adolescent Program here at Grandview. In January 2011, the Board of Directors agreed to reduce the eligible age for clients to access therapy services in this program.
Grandview has been providing medical and therapy services for clients with physical disabilities until their 21st birthday. To bring our programs in line with other Children's Treatment Centres and funding requirements from the Ministry of Children and Youth Services, Grandview has now reduced the age for service to 19.
Clients who are turning 19 years old in 2011 or those who are already 20 are eligible to stay on caseload till their 21st birthday. All other clients will be discharged at age 19 (prior to their 20th birthday). To remain on Grandview's caseload, clients must have active goals and access services at least once in a 12 month period. New referrals will not be accepted for clients who are over the age of 17.
Therapists will work closely with clients and families to help with the transition to adult services. It is important for clients and families to link with the adolescent team to ensure that any outstanding equipment, clinic appointments, and treatment can be completed prior to the age of discharge.
Adolescents participating in recreation programs advertised in the recreation services brochure may participate until they are 21 years of age. |

MAKING A DIFFERENCE IN DURHAM |
|

With the expansion of the Family Resource and Disability Awareness Programs at Grandview Children's Centre, families of children with special needs will be afforded a listening ear as well as a voice.
A recent grant from the RBC Foundation of $21,000 allows for a Family Facilitator with personal experience to connect with families on a one-on-one basis, offering support and advice.
While the Family Resource Centre contains numerous resources for families in-house, the Disability Awareness Coordinator is available to make presentations directly to schools or groups needing information on dealing with special needs and or disabilities. These presentations help increase understanding, awareness, and acceptance. |
