Where Would You Like a TapRooT® Course in 2011?I'm preparing our public TapRooT® Course schedule for 2011 and I'd like your help. If you would like to attend a course or if you have some people to
send to a public TapRooT® Course, I'd like to get your ideas about where to hold the training. So let me know the following by leaving a comment to this article: 1. Where would you like to take the course? 2. What kind of course? (2-Day, 5-Day, Equifactor®) 3. How many people would you plan to send? Please let me know ASAP and I'll get the plans set for 2011. Again, here is the link to leave your thoughts on the above questions: http://www.taproot.com/wordpress/2010/07/06/where-would-you-like-a-taproot%C2%AE-course-in-2011/Thanks for your input! | Investigations and Accidents/Lessons Learned | Accident Implications: How Far-Reaching are the Consequences?  A disaster in one industry can re-open inquiries into accidents that
happened a quarter of a century ago, even in other industries (Read more).
UK RAIB Accident Report on Train Collision at Exeter St Davids Station:  (Read report.)
Fatal Tram Pedestrial Accident Investigation is an Interesting Investigation and Good Recommendations:  (Read investigation report.)
The Washington Post Reports: "Since Red Line Crash, Efforts to Improve Metro Safety Have Lost Momentum":  (Read story.)
Joint Coast Guard & MMS Deepwater Horizon Joint Investigation Web Site:  I'm slow to find this site but there's lots of information for anyone who wants to learn from the accident (Read more).
Cost of an Accident: Costs Sometimes Go Far Beyond the Company Involved:  The six month deepwater drilling ban is already hurting the economy around the Gulf of Mexico (Read more).
Root Cause Analysis Report about a Tritium Leak at Vermont Yankee Nuclear Power Plant is Released:  (View summary and pictures.)
Do Exxon and BP Take Different Approaches to Risk?  A story by Elizabeth Souder of The Dallas Morning News seems to imply as much (Read more).
What is an Accident?  After I started writing about the BP/Transocean Deepwater Horizon
accident, I've had several people write me that I should stop using the
term accident because it implies there is nothing that can (or could
have) been done to stop "an accident" from occurring (Read more).
Are We Blaming BP Rather Than Learning From What Went Wrong?  Most investigators agree that blame makes an accident investigation
more difficult. If someone thinks they are going to be blamed (and
perhaps fired), they often provide less information than if they were
in a non-blame environment (Join ongoing discussion).
CSB Press Release: CSB to Investigate Root Causes of BP Deepwater Horizon Blowout Accident:  Here's a link to the letter by Chairman John Bresland (link).
CNN Reports: Frustrated locals Not Waiting for OK to Stop Oil:  (Read report.)

The Building Blocks of a Good Quality Program ... Is It in the Name? by Chris Vallee
  |
TQM, TQC, TOC, PDCA, Six Sigma, Lean, Lean Sigma, MBO, 8D ... just to
mention a few Quality Programs many in the world of Quality have been
exposed to ... but is it the name of or the effectiveness of the process
that make a good Quality Improvement Program? Seems like a silly
question until you have lived in the world of change.
... "Six Sigma is not the same as TQM"
... "Lean Six Sigma is definitely better than Six Sigma"
... "Is it a Lean Project or a Six Sigma Project?" (Read article.)
