President's Question
mark1Can Regulators Stop Accidents?

The MMS (Minerals Management Service) has been severely criticized for not being a more forceful regulator. The President has already taken action to split the agency in two (one part to collect government oil royalties - the other to regulate safety). But critics in the press are calling for heads to roll.

Even one of the most forceful and independent regulators, the US Nuclear Regulatory Commission, can't stop all nuclear incidents (for example, the Davis-Besse reactor vessel head corrosion hole near-miss).

This brings up a bigger issue ... can regulators stop accidents? What should they be expected to do? Is a cooperative regulatory environment better or worse than a contentious regulatory environment?

To me, the responsibility to stop accidents is held by the management at the company doing the work. In the case of the BP/Transocean Deepwater Horizon accident that responsibility would belong to BP and Transocean. But I can see the roll of regulation is high-hazard industries.

What do you think? Was the MMS too easy? Did a lack of regulation contribute to the BP/Transocean Deepwater Horizon blowout? Is the US government partially responsible for this accident?

Let me know your ideas by leaving a comment at:

Consume Accountability
by Capt. George Burk
Outstanding leaders know that accountability is vital and it extends to other internal core values such as honesty, competence, courage, compassion and honor.  Outstanding leaders "wear" these core values like a coat draped around their shoulders because the core values are first and foremost personal, then professional ... inside out, not outside in.  Here are a few ways to "wear" those core values (traits) (Read article).

Editor's Note:  Let's Get Critical!
Dear Readers,

Have you ever wanted to leave a disagreeable comment to a blog post but didn't?  I've been in that situation before.  Sometimes it was because the blogger seemed so much bigger than life, I thought he or she must surely know more than me.  Other times it was because I didn't want to offend anyone or get into a big messy debate.

Sadly, when a blog's entire readership appears to be nodding in agreement ... it loses its edge.  It becomes a (yawn) Society of Lovely Flowers and Affirmations instead of a vibrant thought producing community

I'm not trying to be a troublemaker here but let's get critical:
  • We want to earn your respect by giving you a forum on the Root Cause Analysis Blog to state your point of view whether it agrees with the point of view of our authors or not.
  • We want to hear your critical feedback to improve any problem we may not have noticed.
And most of all
  • We don't want to lose you because you thought we weren't listening or we don't care what you think.
We appreciate your support and positive comments, and always stay true to your convictions.  Constructive criticism is helpful.  Together, we'll continue "Changing the Way the World Solves Problems."

Best regards,

Barb Phillips
