 Use of the Self-assessment Tool/Audit Report (STAR) Discontinued 

Pursuant to a recommendation made earlier in 2015 by the Accreditation Committee and approved by the Board of Governors, the American Association of Tissue Banks (AATB) has discontinued use of the Self-assessment Tool/Audit Report (STAR) as well as the need for AATB to pre-approve an audit tool if different from the STAR. To maintain consistency for membership, each version of the STAR has been replaced with the relevant Pre-Inspection Checklist (i.e., for a Tissue Bank, for a Tissue Distribution Intermediary, and for a Non-transplant Anatomical Donation Organization).
 
Updates to affected standards, policies and guidance documents are taking place and will be announced in early 2016. The following will be revised accordingly:
  • Standards for Tissue Banking: see B1.521 Inspections/Audits of Other Facilities, B2.150 Quality Assurance Program, and K5.000 INTERNAL AUDITS
  • AATB Guidance Document: Recovery Partner Audit Tool [No. 6, version 1, September 1, 2011]

  • Standards for Non-transplant Anatomical Donation: see NT-B1.520 - NADO Audits of Other Facilities
  • Accreditation Policies for Non-transplant Anatomical Donation Organizations: see I. Accreditation Program - General Provisions at part D., II. Required Elements at D., and VIII. The Accreditation Process at B. and D. 6.
