FormsNet3 Spring 2016 Release
Enhancements were made to the attachment feature. Documents can now be attached at the CRID level in addition to the form level. FormsNet3 will also be supporting additional file types (.jpg, .doc, xls, etc.) which means that they can be attached and viewed. For additional information on the attachment feature, see page 25 of the FormsNet3 Training Guide, or ask your center's assigned CRC.

Learning Center Accounts
When a NEW USER is creating an account:
− Indicate your Primary Center Type as - Transplant Center
− Indicate your Secondary Center Type as - CIBMTR/Research Center
− In the Group Selection section, select the checkbox next to Network.
Allow two business days for your new user account registration to become active before you can access the training. If you still do not have access after two days, please contact the CIBMTR Training Specialist - lcolt@nmdp.org

Question:
Where would positive BCR-ABL findings (by FISH) be reported on the Post-TED?
Answer:
Report this finding in Q86 of the Post-TED (Form 2450), relapse/progression detected by cytogenetic/FISH method. Molecular studies, flow cytometry, cytogenetic studies and fluorescent in situ hybridization, and clinical/hematologic assessment are used to evaluate disease status. Indicate if the disease status was assessed using at least one of these methods at any time within the reporting period. In most cases, the patient will have had at least a clinical and/or hematologic assessment if they were evaluated by a physician during the reporting period. If no assessment was performed during the reporting period for which the form is being completed, continue with the signature section at the end of the form.
