Educational modules are continually being developed to instruct both new and experienced data managers on data collection and reporting.

In May, the CIBMTR began using the FormsNet Query Function to request data corrections whenever possible. For corrections related to ongoing studies, your CRC will still be sending reminders to check for queries, as these projects have specific deadlines. However, CRCs will not be contacting centers when queries are added for other types of data corrections, like: mismatches in the date of birth, sex, or chronological transplant number reported on various forms.

Your center should be developing a process whereby you are using the Center Forms Due feature to search for any forms in QRY status and responding to any queries that have been requested.

MORE NEWS . . .The CIBMTR has completed Form Revision Phase 2 which will be released into production this year.
  1. Cellular Therapy Forms (4000, 4100, 4006), VOD form 2553, and Indication form 2814 in Summer.
  2. Post-TED 2450, CRF Follow-Up Form 2100, Death Form 2900, and HLA form 2005 in Fall. 
With the release of the newly revised forms you will see several major changes. For example DCIs will be captured on the new Cellular Therapy forms.  The CIBMTR will begin to collect chronic GVHD data using the 2014 NIH criteria. More information will be provided as we get closer to the releases. 
CIBMTR® (Center for International Blood and Marrow Transplant Research®) is a research collaboration between the National Marrow Donor Program®/Be The Match® and Medical College of Wisconsin.

FormsNet3 Spring 2016 Release
Enhancements were made to the attachment feature. Documents can now be attached at the CRID level in addition to the form level. FormsNet3 will also be supporting additional file types (.jpg, .doc, xls, etc.) which means that they can be attached and viewed. For additional information on the attachment feature, see page 25 of the FormsNet3 Training Guide, or ask your center's assigned CRC.

When a NEW USER is creating an account:
− Indicate your Primary Center Type as -  Transplant Center
− Indicate your Secondary Center Type as - CIBMTR/Research Center
− In the Group Selection section, select the checkbox next to Network.

Question:  
Where would positive BCR-ABL findings (by FISH) be reported on the Post-TED?

Answer: 
Report this finding in Q86 of the Post-TED (Form 2450), relapse/progression detected by cytogenetic/FISH method. Molecular studies, flow cytometry, cytogenetic studies and fluorescent in situ hybridization, and clinical/hematologic assessment are used to evaluate disease status. Indicate if the disease status was assessed using at least one of these methods at any time within the reporting period. In most cases, the patient will have had at least a clinical and/or hematologic assessment if they were evaluated by a physician during the reporting period. If no assessment was performed during the reporting period for which the form is being completed, continue with the signature section at the end of the form.
