WELCOME DR. DONNEY JOHN   

Donney John, PharmD
Don is one of Pharmacist Partners' key advisors, and is leading our Transition in Care Program. He is a practicing pharmacist, healthcare consultant and entrepreneur.

Don has expertise in patient engagement, population health management, transition in care and mobile health technology solutions. He serves as a medical advisor for a variety of companies focused on health/wellness, patient engagement and medication adherence.  

Previously, Don was an Assistant Professor of Pharmacy Practice at Massachusetts College of Pharmacy & Health Sciences where he developed innovative ambulatory care practice sites within federally qualified community health centers. He has trained pharmacy students, pharmacy residents, medical residents and other healthcare professionals in the areas of health literacy and cultural competency.

He holds a Doctor of Pharmacy degree from St. John's University and completed a pharmacy residency with Massachusetts College of Pharmacy & Health Sciences.

Don is the Interim Executive Director for NovaScripts Central, a non-profit pharmacy that provides free medications to uninsured patients in the northern Virginia area. See more about NovaScripts below.

Getting you on track for transition in care planning

Pharmacist Partners is here to help Hospitals and ACO's incorporate medication reconciliation and other best practice principles into successful discharge planning and patient management programs.

Pharmacist Partners' keys to success for hospitals and health systems:

  • Observe current hospital discharge planning and medication reconciliation workflows.
  • Evaluate opportunities to increase efficiencies within existing hospital workflows and resources.
  • Develop processes to incorporate discharge planning and medication reconciliation best practices
  • Provide hands on training for successful implementation and ongoing training

Dr. John is leading our Transition in Care Program.

PQA  - PHARMACY QUALITY ALLIANCE
Pharmacist Partners has joined the PQA
The PQA is a  multi-stakeholder, consensus-based membership organization that collaboratively promotes appropriate medication use and develops strategies for measuring and reporting performance information related to medications.

The PQA has several workgroups in progress, and Pharmacist Partners has joined four of them.  We are looking forward to contributing to the critical work being done by PQA.  We hope to see you at the November 12 Leadership Summit in Washington DC.

NOVA ScriptsCentral is a pharmacy unlike any you've ever known

It is a nonprofit, collaborative pharmacy, providing life-saving medications for uninsured children and adults throughout the Northern Virginia region. Its safety-net clinic partners provide the health care, and NOVA provides the medication support and the pharmacist counseling.  

 

$30 million of medications have been provided to the most vulnerable in our communities and they've only just begun! It's the only one of its kind in the state of Virginia and in the metropolitan region, and is a national model for pharmaceutical care for the uninsured.

 

NOVA Scripts is cost-effective. The model is based on research of best-practices and focuses on economy of scale: many clinics sharing one pharmacy for the benefit of all, rather than each clinic developing their own pharmacy.
