Required System Performance Measures KYHMIS Data Clean-Up Deadline
The U.S. Department of Housing and Urban Development (HUD) System Performance Measures include a set of seven measures that each Continuum of Care (CoC) must submit to HUD annually. The set includes:  total number of people experiencing homelessness, length of time people spend experiencing homelessness, percentage of people who leave homelessness to permanent housing, number of people who return to homelessness, and more. KYHMIS Annual and Required Documentation
Invoicing
Every year, Kentucky Housing Corporation (KHC) invoices for the next fiscal year's KYHMIS User License fees. In the 2015-2016 fiscal year, online payments were introduced to the KYHMIS agencies.

KHC has adopted this method of invoicing permanently. During the last week of June, invoices will be emailed to each agency for fiscal year 2016-2017 billing. All agencies in the BoS and the Lexington CoC that participate in KYHMIS and have users with licenses, will receive an invoice for their portion of the license fee.

The Louisville CoC will bill their Louisville CoC clients directly and then pay their portion of the license fees (for all Louisville agencies) directly to KHC.

Each BoS and Lexington CoC agency must review the number of current KYHMIS users and plan for the number of KYHMIS user licenses they will need for the upcoming fiscal year. Your agency will be charged for the number of users listed. Please remember that user licenses cannot be shared by more than one user. Refunds will not be given for licenses requested in July, which weren't used consecutively during the year.

Agencies may access the PNC Payment Center to log in or pay now. If you have any questions, please contact the HCA Help Desk toll-free at 800-633-8896 or 502-564-7630, extension 446; TTY 711; or Submit a Request through the HCA Help Desk.

Required Documents
All agencies are required to submit the following documentation each July. If a new user has recently been added to the system, the paperwork has to be re-completed for July and will be in effect for the entire fiscal year.

The documents are available on the HCA Help Desk, under KYHMIS Administrative Documentation, Agency Related Forms.
  • The KYHMIS Agency Participation Agreement
  • The Security Monitoring Form
  • The KYHMIS User Confidentiality Agreement (one per user)
  • The signature pages of the KYHMIS External Policy and Procedures (one per user).
The Executive Director should sign where indicated. Once completed, the forms need to be scanned and submitted to the HCA Help Desk.
Aspen Dental's National Day of Service
Thousands of veterans will receive free dental care on Saturday, June 25, when doctors and their teams from nearly 400 Aspen Dental-branded practices in 33 states open their doors for Aspen Dental's National Day of Service. This event is expected to be the largest single-day oral health initiative for veterans in 2016.

Like millions of other Americans, veterans can struggle to find oral health care when they need it. That's why Aspen Dental's Healthy Mouth Movement, a community-giving initiative launched in 2014, is focusing its efforts on helping veterans get the dental care they need.

Here is how you can help:
  • Interested veterans should call 1-844-AspenHMM to find a participating practice in their community and schedule an appointment in advance - space is limited and appointments are filling up fast!
  • Please spread the word in your community!  Be sure to use #HealthyMouthMovement and tag Aspen Dental when posting on social media.
