Greetings!

We have some exciting updates to share with you!
Patients Assisted Last Spring marked a new beginning for Carmella Rose Health Foundation when we broadened our navigation outreach throughout the Cleveland area by hiring our first Community Patient Navigator, Ginny Pate. Since April 2012, she has helped over 550 Northeast Ohio patients access quality care and services by providing one-on-one guidance, support, education, referrals, coordination of care, and other assistance.

types of assistance
The patients we serve through the Community Navigator Program are primarily uninsured and consistently need help finding low-cost prescription programs to pay for their medications, finding a wheelchair or other medical equipment, getting dental care or vision exams, and much more.

We invite you to read the complete 2012-2013 Patient Navigator Report
to learn more about the people we serve in Northeast Ohio and the types of assistance we provide.
Please Help Us Continue Helping Others!

Because the services provided by the Carmella Rose Health Foundation are free, we rely on the support of donors like you to continue helping the patients we serve.

When you support the Carmella Rose Health Foundation, you touch the life of someone who really needs help navigating the complex healthcare system in Northeast Ohio, like Marie.* Marie's neighbor Mike* was at the food pantry where he met Ginny Pate, our Community Patient Navigator. Mike was concerned because Marie's doctor felt a mass during her annual physical examination. Marie had recently lost her job and did not have insurance, so she didn't know how she was going to pay for the diagnostic mammogram that her doctor recommended. Ginny was able to help Marie get a free mammogram so she could follow-up
without delay. Read other examples of the patients we've helped in the complete
2012-2013 Patient Navigator Report.
