We are very excited to share the news that COLLAGE will launch a new enterprise that uses art and science to promote optimal wellness. The program combines an award-winning, innovative senior coaching process called Vitality 360 with the power of information derived from the COLLAGE assessment system. Inevitably, some of you will have immediate questions - below are a few we anticipate with answers following.
Why are the two programs being integrated into one product now?
Kendal and Hebrew SeniorLife, the program collaborators, recently decided that COLLAGE would be significantly enhanced by integrating it with the Vitality 360 senior coaching process. Orchard Cove, a COLLAGE member since 2007, developed and launched Vitality 360 in 2009, refined the program, and in the span of three years achieved remarkable results. We believe for COLLAGE to thrive we need to bring the two programs together into a seamless product for all current and future members.
What are some of the outcomes that Orchard Cove has achieved with Vitality 360 and COLLAGE?
Orchard Cove has found significant positive results, including:
- 90% of residents are participate in the program
- 85% of the participants developed a fitness plan
- Of those resident participating, 80% exercise on a regular basis (more than twice a week)
- Since 2011, the percentage of residents who exercise increased from 33% to 74%
- 89% of the residents participating reported having good or excellent health - up from 80% one year earlier. This was a statistically significant finding.
- 62% of the residents participating stated they are delighted with life as a whole - up from 32% one year earlier. This was a statistically significant finding.
What are the core components of the new program?
360º View
The new program does not look at just one part of an individual. It looks at the entire well-being of a person - the 360 degree view - and encourages residents to consider all aspects of their lives including health, nutrition, physical and mental fitness, community activity, social connections, spiritually and lifelong learning. Through a coaching conversation, the resident is supported and encouraged to think about their life and legacy, and create a full, vibrant lifestyle.
Coaching Model
The coaching model focuses on engaging the residents in meaningful conversations that facilitates self-direction and action. Residents develop a personalized vitality plan that often inspires lifestyle changes. The coach is trained to listen and observe, and completes a subtle, non-invasive evaluation using the Health and Social Check-up tool. Both the vitality plan and evaluation data are tracked for resident follow-up and can be utilized for organizational outcome measurements, benchmarking and quality improvement.
Interdisciplinary Team
The interdisciplinary team (IDT) - which may include fitness and social service staff, chaplain, member services representative, nursing, medical practice physician - is guided by the coach, and serves a critical support, resource and advocacy function. In many cases the IDT's primary organizational role and function are completely redefined to be able to support the new program. The coach communicates on a regular basis with the resident about his/her individual vitality plan and the interdisciplinary team discusses and consults with the coach on a regular, prescribed basis.
Evidenced-based Assessment Tool
The new program has at its foundation an evidence-based (i.e., scientifically-grounded) assessment system that includes the Lifestyle Survey and Health and Social Check-up. This means that organizations can have confidence that the assessments are valid and reliable and have research to stand behind them.
What is the timeline for launching the new program?
Starting in February 2013 we will begin to train our current consortium members and this scope of work will be completed by June. We anticipate offering a robust product in late 2013.
Integrating COLLAGE and Vitality 360: Addressing Cost, Outcomes, Experience of Care
The Institute for Healthcare Improvement's (IHI) "Triple Aim" framework for optimizing healthcare system performance includes reducing cost, improving outcomes, and improving the experience of care. An aging services community is a microcosm of the larger healthcare system, and the new program lays the groundwork for the necessary infrastructure and operational change needed to optimize your health/wellness program.
