Wait.  Are Hospitalists increasingly present in subspecialty services, or are some specialist services reorganizing according to the hospitalist model?  Um...Yes and yes again!

How long have you been a Hospitalist?  Since 2001

 

Do you have a particular specialty or area of interest within hospital medicine?  

Like all hospitalists, I'm a generalist, but I have a few areas of clinical interest, including inpatient diabetes management, the evaluation of patients with syncope, and judicious test ordering. In addition, I have several areas of interest that coincide with my research, including medication reconciliation, improving the hospital discharge process, improving handoffs in care within the hospital, and improving communication between patients and providers.

 

Favorite apect of your job/profession? 

I am constantly inspired by the talents, dedication, and varied interests of my colleagues. I also love the day-to-day flexibility of my job: no one is in the waiting room, and I can spend an hour or two with a patient and his/her family if that is what they need. And while patients get my full attention when I am on service, the job lends itself very well to doing other things, like research and hospital operations when I'm not on service.

 

Least favorite?

The frustrations of working within a fragmented health care system that often makes penny-wise, pound-foolish decisions. That is one reason why I look forward to the changes already underway to create a system based more on providing value and less on maximizing the volume of services provided.

 

If you could change one thing about the state of hospital medicine today, what would it be? 

 

That more hospitalists were trained in quality improvement and were then given dedicated time by their hospitals to do this work, given the support they need to succeed (like access to data, project management assistance, and QI experts), and some measure of accountability for success. Quality doesn't happen by fiat at the C-suite level - it takes an army of providers who practice it every day, and hospitalists should be a major part of that.
