We have many patients who come in to find better alternatives to their current PAP masks. And more often than we would like, these patients are struggling to adjust to PAP therapy overall.
The good part is that these people are trying to make it work because they realize that the consequences of not using PAP therapy are worse.
Those of you who have been using PAP for some time know that there are a variety of reasons why it is hard to live with:
Masks are uncomfortable and in many cases cause pain or create sores on the skin; masks don't fit well and air leakage makes it nearly impossible to use; exhaling against pressure is uncomfortable; wearing the mask is embarrassing and so NOT sexy....etc. The list goes on and on. For these issues, we encourage people to be patient, find a place where you can try a variety of masks and get expert advice, and find support from others who use PAP.
The people we are focusing on in this article are those who suffer from claustrophobia and those who require higher pressures but have a difficult time tolerating it. For these people we recommend a desensitization protocol to help you overcome your fears and/or discomfort.
For some, desensitization means putting the mask up to their face for a few seconds, then taking it off. When they can have the mask on for a whole minute, it is time to try attaching the head straps, even if only for a few seconds. All of these steps are accomplished while the PAP machine is OFF.
In general, all steps should be approached at the person's own pace. And a person can begin desensitization at any level of the process. For example, if you do not have a problem with the mask being on your face, you might just start with putting the mask and headgear straps on all at once and wear the mask for a few minutes without pressure.
Once the person is acclimated to the mask, pressure can be added at the lowest setting. We recommend that the person be engaged in some other activity while using the pressure, such as reading or watching TV, so that the focus is not on the mask and pressure. This can be done for just a few seconds/minutes, working up to longer times. Eventually, you just might fall asleep with it on and wake up the next morning refreshed!
Keep in mind that many insurance companies require that you show proof that you have been using your PAP for so many hours and so many nights per week. Failure to do so may result in the loss of coverage for your equipment. If you are a person who will need to use desensitization protocols to accept PAP therapy, consider renting a machine for a period of time before getting your insurance involved.
