NOTES FROM 215:
|
Starting with the 2013-2014 school year, all students in grades 6 through 12 will be required by Illinois state law to show proof of having received the Tdap (combined tetanus, diphtheria, and acellular pertussis) vaccine. The Illinois Department of Public Health issued this new requirement in response to an increase in pertussis, also known as whooping cough, among adults and children in our area. If your child has not already received the Tdap vaccine, he/she will be required to receive it before August 1, 2013. Students will not be able to enroll in school, in August, without the Tdap vaccine, unless they have an approved medical or religious exemption on file with the school.
or (708) 585-2050
or (708) 585-1009
regarding this requirement BEFORE May 31st.
The school nurse does not have regular hours during the summer and will not be available until registration/book pick up, in August, for review of your student's Tdap documentation. Please take care of this matter NOW, so as to avoid long lines at book pick up in August.
All parents/guardians of current sophomores (Class of 2015) and juniors (Class of 2014) MUST contact the school nurse:
All current freshmen (Class of 2016) are already in compliance with the Tdap requirement, so there is no need to contact the nurse regarding these students.
Acceptable documentation of the Tdap vaccine includes:
- A signed note or letter from a physician indicating the date the vaccine was given
- A printed copy of your child's electronic immunization record
-
A copy of his or her school physical form that specifies the exact date of the vaccine.
- If any corrections or additions are made to a previously dated form, it must be re-signed by the doctor with the current date
