DEPENDENT ELIGIBILITY VALIDATION MONITORING UNDER WAY

The State Health Plan has begun dependent eligibility validations with HBRs to verify that proper documents are being requested and validated on employee dependents.

If this validation determines that an employee has not submitted the proper dependent eligibility documents, the employee has two weeks to submit the proper documents to the HBR. If after two weeks the proper documents are not received by the HBR, that employee's dependent(s) will be termed from the Plan.

Thank you for your cooperation. Please remember that the Plan will contact the HBR's office concerning recent employee life events and eligibility documents.

PREMIUM PAYMENT DUE DATE

As a reminder, the premium for the State Health Plan is due on the first of the effective month. Premiums that are not paid by the fifth of the effective month are considered past due and claims will be placed on hold until the premium is paid in full.
