On Friday, June 26th Healthy Transitions Colorado held its very first symposium: Building Transformation from the Inside Out. The symposium was the first in a series of three with a complementary learning series titled Cultivating the Network of Care. Thank you to everyone who made the event such a success! Over 70 partners came out for networking and learning more about how to prepare their facilities for community-based care.

Aurora's Underserved and Anschutz's Hopefuls Come Together at DAWN Clinic

University of Colorado's Anschutz Medical Campus is a dominating feature of the landscape in North Aurora. The world-class facility, providing world-class care, is surrounded by one of the most economically depressed communities in the state. Using the resources of the Anschutz Campus, the newly-opened DAWN Clinic is working to combat the disparity in health care for the area's vulnerable citizens.

Study of Returns to the ER Suggests Lack of Follow-Up Care By Lauran Neergaard, June 2015 No one wants to make a repeat visit to the emergency room for the same complaint, but new research suggests it's more common than previously thought and surprisingly, people frequently wind up at a different ER the second time around. Hospitals are looking into video calls to address patient's concerns prior to another visit and electronic sharing of patient information among other options.

Preventable Hospitalizations Down Among American Seniors

The shift from fee- for- service to a value based payment system is one of the reasons why there has been a reduction in senior hospital readmissions. An overall greater focus on population health including preventive services such as getting the flu shot also contributes to reducing senior hospital admissions and readmissions.

New Readmission Reduction Guidelines: Medication Management Essential

Better management of a patient's medications and improved medication adherence are essential to reducing hospital readmissions. Communication and face-to-face interaction are still at the forefront of achieving those reductions. This article highlights six recommendations for hospitals.
