Healthy transitions. Two buzzwords mentioned a lot these days due to health care's focus on reducing hospital readmissions. The coordination of care is crucial to ensuring that care transitions are effective and safe. Healthy Transitions Colorado (HTC) brings providers of care together for a collaborative and collective impact on how to best coordinate those transitions. Helping to break down barriers in communication and foster relationships is work I am passionate about and, knowing I wanted to be a part of this initiative in Colorado, ultimately brought me to Healthy Transitions Colorado.
