Focus: Community and Care Transitions
Build It and They Will Come: Senior Housing Needs in Colorado, U.S.

A couple months ago the Joint Center for Housing Studies of Harvard University released a report finding much of the country isn't prepared to meet the housing needs of older adults.

 

This issue has been with us for years, so it's no wonder in the short time since the report was released nothing has changed.

 

High-quality, affordable housing for older adults remains scarce in Colorado and throughout the nation.

 

Much of what is available isn't easily accessible for those with physical issues or for those who use a wheelchair. Simple things many of us take for granted-the ability to grab a door knob-can't be accomplished by some older adults.

the Power of Community to Drive Improvement

The Commonwealth Fund has developed an initiative that examines communities with significant poverty that nonetheless perform in the top tier on a composite measure of health care access, quality, efficiency, and outcomes. The case studies can be used to inform new developments in delivery system changes and payment reform to improve care across all communities.

Systems Serving Low-Income Patients Struggle Under Readmissions Penalties

A report published in the Joint Commission's Journal on Quality and Patient Safety found that readmission penalties have a disproportionately high impact on low-income hospitals. Hospitals that may already be under-resourced due to the socio-economic mix of their patients, have a harder time coping with the readmissions penalties than larger, more financially stable hospitals do.

Silver Tsunami Poised to Overwhelm Home Care Workers
