Phil preferred photoIt's hard to believe that this will be my final blog post as President and CEO of CIVHC.  By the time our next newsletter rolls around, I will be in the same role but with Pinnacol Assurance, Colorado's leader in worker's compensation insurance. The work I've been engaged with at CIVHC, together with our many partners, has been some of the most challenging and gratifying of my career. I'm proud to have played a small part in joining with so many caring individuals and organizations who are united in helping Colorado achieve the Triple Aim of better care, better health and lower costs.

For the first time since the Blue Ribbon Commission for Health Care Reform (the 208 Commission) developed its recommendations for comprehensive health reform in 2007-08, Colorado is thinking big about how to transform our health care delivery and payment system.

Spotlight on Innovation: MCPN Bridges to Care Hot Spotting Project

Across Colorado and the US, roughly 20 percent of the population is responsible for 80 percent of the health care costs (www.cohealthdata.org). This disproportionate amount of spending on a few has led to what's been coined "hot spotting" - care coordination programs targeting high-risk, chronic condition patients aimed at reducing unnecessary ER visits and hospital stays. As sub-recipient of a federal innovation grant through the Center for Medicare and Medicaid Innovation (CMMI), Metro Community Provider Network (MCPN) in Aurora, CO, has seen over $145,700 in estimated cost-savings and a 63 percent reduction in utilization of hospital and ED services through their hot spotting care management initiative.

Healthy Transitions Colorado, a statewide umbrella campaign to coordinate and advance care transitions programs across the state, received a Bronze Gold Leaf Award in the website category at this year's Colorado Healthcare Communicators Gold Leaf Awards. As managing partner of the campaign, CIVHC would like to thank and congratulate our care transitions task force and operating partners CFMC, CRHC, CHA and COHCA for their devotion to creating a site focused on engaging organizations and supporting efforts in this important work. Special thanks to our web designer and developer, Andy Campbell Creative.
