On March 12, 2015 patients, families, nurses, physicians and caregivers came together to discuss the important topic of youth transitions to adult care. The group discussed three Canadian transition programs and discussed a strategy for building and implementing an effective approach in transitioning youth with chronic health conditions to adult care in Northern Alberta. Results from the Symposium have been compiled and are available.
The Transitions of Youth to Adult Care Steering Committee has reviewed the report and as a result is expanding its membership to include representatives from the Glenrose Rehabilitation Hospital, Family Medicine and Primary Care. Further information will be shared once the expanded Steering Committee has had a chance to more thoroughly review and discuss the recommendations of the report. A learning collaborative model will be used to develop and implement the transition program. The learning collaborative approach is designed to provide the opportunity for clinical teams to learn together, work together and share experiences to move toward improvement.
