Transition of Youth to Adult Care Symposium

Round Table Discussion On March 12, 2015 patients, families, nurses, physicians and caregivers came together to discuss the important topic of youth transitions to adult care.  The group discussed three Canadian transition programs and discussed a strategy for building and implementing an effective approach in transitioning youth with chronic health conditions to adult care in Northern Alberta. Results from the Symposium have been compiled and are available.


The Transitions of Youth to Adult Care Steering Committee has reviewed the report and as a result is expanding its membership to include representatives from the Glenrose Rehabilitation Hospital, Family Medicine and Primary Care.  Further information will be shared once the expanded Steering Committee has had a chance to more thoroughly review and discuss the recommendations of the report.  A learning collaborative model will be used to develop and implement the transition program. The learning collaborative approach is designed to provide the opportunity for clinical teams to learn together, work together and share experiences to move toward improvement.

Family Bedside Orientations
Some of our amazing volunteers have continued to visit families on unit 5G4 at the Stollery.  The second quarter report has just been released and it shows how impactful and meaningful those visits have been. 
 
62 peer support visits with an 80% positive rating of the visit. Read more about this and other facts from the report.

The Family Centred Care Network is supported by the Stollery Children's Hospital Foundation.  Donors contribute to the Foundation to help improve the health care experience for patients and families at the Stollery Children's Hospital.  

We'd love your support to help us share messages relevant to all the wonderful work happening in the different areas of the Family Centred Care Network. A few quick tips to keep in mind before you post:

  • everyone acting on behalf of AHS must use good judgement in their public interactions
  • make sure you have appropriate permission to post pictures of others 
  • avoid giving your opinion/advice as a volunteer or employee of Alberta Health Services
  • protect patient, employee and others confidentiality
  • what is shared on social media is accessible to a broader audience
  • once something is on the web, it is out of your control
  • know your privacy settings and how/when they work
  • consider what you are posting before you send it
