| Stollery Children's Hospital 2030 Vision Survey | 
We want to hear from you!
Site Master Planning for 2030 has been occurring across the Edmonton Zone. The Stollery Children's Hospital would like to start the initial steps of creating a vision.
True visioning takes time and requires multi-stakeholder feedback. We want to ensure we have included the voice of what staff, physicians, patients and families think our vision might include.
We want to hear from as many people as possible. We are gathering your words of wisdom from November 3-17, 2015.

| PICU/PCICU Quality Council |
Provides an opportunity to discuss operational issues with families and healthcare professionals outside of a care moment; when both the family and health providers are in a more reflective space, at a distance from providing or receiving care. We are currently looking for 2 family members to participate on this quality council.
This role requires creative solutions, family perspective, sharing of your story and the ability to use these skills and experiences to generate quality improvements in council projects. This will be achieved by:
- clarifying possible assumptions about what patients, and families want by sharing own experience and that of other families
- sharing stories to create understanding and provide context
- providing the family perspective in the review of materials and processes
- encouraging and recognizing the staff for the good work they are doing
- Communicating issues and results with other patients and families (electronic / virtual family group, FCC Council) while maintaining the rules of confidentiality
You should consider this opportunity if you:
- have experience with a child / youth in the PICU or PCICU,
- are a big picture thinker,
- are able to share your story and personal experience in a constructive manner,
- are passionate about making a positive change for the future.
You will be required to commit to 4-6 hours per month: at the monthly Quality Council meeting, participate on one working group meeting and engage other families on the Family Centred Care (FCC) Council/Network through

| NEW Cardiology Family Advisory Care Team |
 The Stollery Cardiology program is interested in establishing a NEW Cardiology Family Advisory Care Team (FACT) and is looking for 11- 12 family representatives and 4 front-line staff interested in building the foundations of this exciting new team.
This advisory team would ensure a collaborative dialogue exists between families, staff and physicians in order to improve the delivery of care and operations of the Stollery Cardiology program.
Consider joining the team if you would like to work in partnership with Stollery leadership, staff and physicians to support and inform the delivery of care. Help the Stollery promote family centred care, and provide input from a family perspective to improve the experience of patients and families.
Monthly afternoon or early evening meetings (with the exception of December and the summer months) as well as, committee work, presentation and project involvement. Total time commitment should average 5-8 hours a month.

| Stollery Quality Council | 
The Stollery Quality Council ensures hospital discussions and decisions are being made in the best interest of patients and families. This is achieved through sharing family experiences, reviewing documents and processes as well as, communicating issues and results with other patients and families.
We are currently recruiting 1 family member to sit on the Stollery Quality Council. If you have experience with a child in the Stollery, have been involved in process review or quality improvement measures and want to represent the family perspective then we want you!

Family Talks
| 
The Family Talks program provides an opportunity for families to share their healthcare story for quality improvement, change and improved understanding. Numbers and facts can tell us only so much about why and how change is required; it is the story behind the numbers that builds the connection to the patient journey. Numbers create knowledge and stories provide wisdom; together they create excellence.
The Stollery realizes the value of hearing and sharing patient stories and the requests for families to share their stories is always increasing. Support developing your story, feedback and debriefs are provided to help you achieve success in this role. There are a variety of time commitments, if you are a comfortable public speaker and would be interested in sharing your story for new staff orientation, the education of health professionals or Grand Rounds contact Christie christie.oswald@ahs.ca for more information.

| Pediatric Pain Management Committee |  The committee works to improve pain assessment and management for children at the Stollery. Committee members collaborate on the development and review of documents as well as, sharing their experiences in order to improve practices.
This year the committee's focus is the creation of parent materials so it is critical to have input from parents.
- Family members should have experience with pediatric pain management or have children who have undergone procedures such as multiple IV starts, needles for blood work, or other painful treatments
- Looking for individuals who can attend committee meetings during the day (in-person or virtually via Microsoft Lync) 5x a year

| Peer Support |  Having a child in hospital can be a very stressful and emotional time for family members. Often families find it enormously helpful to speak with someone who has had similar experiences to their own. We are currently recruiting peer support volunteers for our 3 peer support programs supported by the Family Centred Care Network. Ideally peer support volunteers have inpatient experience at the Stollery. To be eligible for these positions applicants must register as a volunteer, obtain a criminal record check and participate in our Peer Support Training Program. Volunteers in this program are supported by a Family Centred Care Coordinator and a Social Worker.
Coffee, Comfort and Conversation - Weekly drop-in family peer support group for David Schiff NICU & PICU families
- Facilitated by Social Worker and Stollery Chaplain with graduate NICU/PICU family member to provide peer support
- Wednesday afternoons, 2:30-3:30 pm in the NICU Conference Room
- Family Mentor time commitment: 1 shift/month
Family Bedside Orientation - One to one peer support program for inpatient families on 5G4, medicine unit
- Visits involve both peer support and orientation information
- Family Mentor time commitment: 1 ½ - 2 hr shift/1-2 weeks, weekday afternoons and evenings
NICU Parent Group - Weekly drop-in parent group for RAH NICU parents
- Facilitated by Social Worker with graduate NICU family member to provide peer support
- Wednesday afternoons, 1:30 - 3 pm in the Bear Den
- Family Mentor time commitment: 1 shift/4-6 weeks
The next Peer Support training is December 7 & 10 from 5:00 - 9:00 PM.

| Maternal Newborn Child & Youth Strategic Clinical Network | 
The Maternal, Newborn, Youth and Child Strategic Clinical Network Committee is looking for 1 family member representative in order to ensure the voices of patients and families are expressed and incorporated.
Family representatives will be involved in system level planning, improving network governance, highlighting areas that need work and implementing work groups to accomplish these strategies.
Representatives should have experience in the maternal, newborn or child areas of health care, applied understanding of the Alberta health care system and an ability to advocate the perspective of patients and families. Most importantly, the representative should have a desire to make meaningful contributions to the delivery of care in the province. This is an ongoing committee and terms run for 2 years. Monthly meetings are approximately 2 hours in length. Meetings will be held in-person and via teleconference. We are making ripples everyday and those ripples have a growing effect. We are getting more requests than ever to hear from families and patients as advisors, partners and consultants.
