Follow-up to May 4 Connections Member Update
PCMH Recognition and Technical Assistance for FQHCs through GRIP

During yesterday's GRIP Open Forum, one of the moderators stated that HRSA grantees must submit a notice of intent (NOI) to participate in the "HRSA Accreditation and PCMH Recognition Initiative" in order to receive funding for the survey tool offered by NCQA GRIP.

Click HERE to begin the NOI. Please note that it will take about 2-3 weeks for HRSA to review your NOI before it's forwarded to NCQA for processing.
New PCMH TA Funding Opportunity from HRSA
Application Deadline - May 31st

In addition to assistance through NCQA GRIP, Health Center Program award recipients can now apply for FY 2016 Patient-Centered Medical Home Supplemental Funding Opportunity Technical Assistance through HRSA (FOA HRSA-16-183). Participation in the initiative mentioned above is not required for HRSA-16-183 eligibility, although HRSA strongly encourages health centers to participate in both the initiative and the funding opportunity.
