Self-Management Programs in Transition
DCAC has promoted the free six-week Stanford Chronic Disease and Diabetes Self-Management Programs, offered by the Atlanta Regional Commission and the Metro Atlanta YMCA, since 2011. Grant funding for both programs ends this summer.
Plans are underway by the Georgia Dept. of Public Health to expand the content and availability of Diabetes Self-Management Programs in local Health Departments. In addition, local Area Agencies on Aging throughout the state are exploring ways of sustaining these programs without grant funding.
Lastly, businesses are now able to provide the six-week online Better Choices, Better HealthR workshop to their employees as a covered health benefit. Canary Health is the provider of this online workshop along with the yearlong Digital Diabetes Prevention Program. Learn more.
