Is Your Facility Tracking Grievances Properly? |
In 2016, the Network will be working on a project to review and foster the development ofrobust and complete grievance logs at the facility level. By reviewing these logs, facility staff can improve provider/patient relationships and be better prepared for a state survey. For more information on what sate surveyors look for in the survey process, please read the Patient grievance/complaint/suggestion system" on page 25 of the ESRD Core Survey Field Manual. |
2015 Critical Asset Survey (CAS) |
Preparing for Emergencies and Disasters |
The CAS is designed to inventory and report information that is essential in the event of an emergency or disaster. Each facility in New York State is required to submit the CAS annually.
On November 16, an email with CAS instructions was sent out to Facility Administrators, Disaster Contacts and Disaster Contact backups, as a reminder for all facilities to complete the CAS for 2015. 2016 Annual Kathe LeBeau Patient Advocate Award |
This award is given to a New York State ESRD patient advocate who exemplifies Kathe LeBeau's tremendous work in the ESRD community on national, state and local levels. Nominations will be accepted until February 14, 2016, and the award will be presented at the Network Annual Meeting on April 20, 2016. Resources from the Kidney Community Emergency Response (KCER) Coalition:
One critical area of concern is for the disaster preparedness and response capabilities of pediatric nephrology patients and providers. Pediatric dialysis patients are more limited in finding treatment locations than adult dialysis patients. This limitation drives a greater need for a higher level of disaster preparedness for this population. Five Diamond: Slips, Trips and Falls |
The 5 Diamond Approach to Patient Safety Program has a module on Slips, Trips and Falls that can help your staff and patients prevent falls. The modules are FREE and technicians can receive continuing education credit if the module is presented by an RN!
The following feedback on the Slips, Trips and Falls module was collected from staff members in facioities that have participated in 2015:
- All falls are discussed at the monthly QAPI meeting. The "Falls Management Investigation Sheet" will be a good tool in conjunction with the incident report to track the incidence of falls. It is a good baseline for a root cause analysis of the event. In addition, after any fall, or potential fall, the staff will discuss what could have been done to prevent this occurrence. This will also be incorporated into the QAPI meetings.
- This module helped us to focus on fall prevention and educate our patients. They now have a voice in their own safety.
- Thankfully we have not had any on site falls, but we do have a patient who falls frequently. His case was discussed, and a referral was made to an outside agency. In discussions with patients they commented on how the floor around the sinks gets wet because of hand washing by the staff. We never realized this as a potential problem so it was good feedback.
- Additional education for patients regarding safe non-slip foot wear and waiting for staff before stepping on the scale.
- Patients were reeducated regarding wearing shoes with open fronts or backs. Patients are to be escorted on and off the scales. Patients also received education on how to prevent falls at home.
- Fall assessments are done upon admission and as deemed necessary when changes are noted in patients' health status. All noted occurrences of falls, slips, and/or trips are discussed during QAPI meetings. All staff have been educated on maintaining a culture of safety at all times. Our facility performs monthly physical plant audits and well as quarterly environmental audits; all of which are reviewed with plans of correction discussed and created when needed.
