Ensuring Data Accuracy
Are you using TWO Needles in a dialysis access? CROWNWeb and NHSN have different reporting criteria, and it is important that you report correctly in each system.  Incorrect data entry can impact your fistula rate.  If you are using TWO needles in a fistula, you report that patient as an A/V fistula patient in CROWNWeb - even if the patient has a catheter in place that is not being used.

The Network's Annual Meeting will be held on April 20th at the Garden City Hotel, in Garden City, NY.  This continuing education meeting provides an opportunity for you and your colleagues to network and share ideas, and will be open to all members of the ESRD community: administrators, dietitians, nurses, patient-consumers, pharmacists, physicians, social workers, surveyors, technicians, and other interested health care professionals. Ensuring Data Accuracy
 
In response to anecdotal reports of intentional non-reporting of infection data, the Centers for Disease Control and Prevention (CDC) and CMS are jointly issuing a reminder that addresses concerns about healthcare facility non-reporting of healthcare-associated infections events. While there is no evidence of a widespread issue, CDC and CMS want to emphasize that accurate reporting to NHSN is critical.
 
CDC and CMS are committed to ensuring data accuracy and reliability. Identifying infections and making sure that patients receive the highest quality of care is a top priority.

Best Practices: Sharps Safety
 The 5 Diamond Approach to Patient Safety Program has a module on Sharps Safety that can help your staff prevent needle sticks. The modules are FREE and technicians can receive continuing education credit if the module is presented by an RN!

Here is provider feedback from the Sharps Safety module from facilities that have participated so far this year:
  • Reinforcing the culture of safety with staff is key in decreasing the number of needle sticks. Staff education needs to be constant.
  • This module has actually increased staff awareness of safety issues i.e. they identified that we need more sharps containers, so we added 12 more. Staff also identified difficulty in engaging the device on the needles used for cannulation. We will bring safety up at all staff meetings this year for further input by staff.
  • Upon performing the assessment worksheet noted we should start injury prevention to be done on annual review and in-service packets.
  • We will start an "accountability for safety assessment" on annual performance evaluations.
