Communication Can Improve Your QIP Score! |
Are you working with your staff to ensure that they are communicating with patients effectively? Have you been working with your patients to help them learn the language of ESRD? The 5 Diamond Approach to Patient Safety Program has a module on Communication that can help your staff and patients. The modules are FREE and Technicians can receive continuing education credit if the module is presented by an RN!
Here are some Communication best practices from facilities that have participated so far this year:
- Upon review of the most recent completed ICH-CAHPS Survey results, an upward trend in our facilities' survey scores is evident. Although the reported scores fall slightly below the benchmark, our staff is encouraged that the programs that have been put into place appear to be having a positive impact on our patients' experience. We feel that this is due in part to the 5 Diamond Communication module as well as a staff education project designed to enhance communication skills, titled "Words That Work".
- From the current results (Oct 2014 through Jan 201515) the ICH-CAHPS summary report reveals that our patients are more satisfied with the nephrologists' communication than with the quality of the dialysis center, care, operations, and providing information to the patients. This clearly identifies the need for improved communication and professionalism among the staff so as to give them the ability to better and more effectively communicate with the patients.
- The results from this ICH-CAHPS survey helps us to focus our efforts to improve communication and professionalism for our staff via this in-service. In this way, our staff can better relate to and teach our patients. We monitor future results of this survey to evaluate our efforts and to determine if further improvement is necessary.
- Our facility will focus on having regular and routine staff meetings to improve communication and to share the happenings of the unit and gather staff input.
- Most areas are improving. Patients don't perceive that we are providing enough information.
- It helps us understand from the patients perspective what they would like and how they feel they are being treated. This can only lead to improvements.

DON'T Be a Security Incident Statistic! |
Protecting patient information is everyone's job. When you communicate with the Network, are you sure you are protecting your patients' information? If you send an email, remember, you are NEVER to send patient information (PHI or PII) or passwords to the Network or the Help Desk. The Network is required to report all security incidents to CMS.
Of the most recent occurrences, there was always a better way to communicate which could have avoided being reported as committing a Security Incident:
* A facility staff member emailed the names of four patients when they could have emailed four CROWN UPIs instead
* Another facility staff member, in response to a request for a CROWN UPI for a patient, emailed the patient name instead. The CROWN UPI is available in the patient CROWNWeb record.
* A facility staff member emailed three patient names and their not yet fully executed consent forms for their participation in a Network project. These forms should have been faxed or USPS mailed to the Network instead.
* A facility staff member emailed PII to the Network, requesting assistance in getting the patient admitted to another facility. This could have been handled by emailing the patient CROWN UPI, or calling or faxing the Network asking for assistance if no CROWN UPI was available.

Registration is now open for the Centers for Medicare & Medicaid Services (CMS) Rollout of Dialysis Facility Compare Star Rating. This MLN Connects™ National Provider Call will provide stakeholders with an update on the following:
- The first year of Dialysis Facility Compare star ratings
- Future plans for Dialysis Facility Compare and the star ratings
