Patient Advisory Committee (PAC) presents
the Medical Education Institute's 
Kidney School™

 

You can enroll for a PAC Kidney School diploma or
join us for any session that interests you!

 

To Earn a Diploma:

PAC/LAN Kidney School Students:

  • NY State PAC Members/LAN SMEs can enroll in Kidney School and will be mailed copies of course materials.
  • Any ESRD Patient/Care Partner or Family Member can attend without enrolling.
  • Upon completion of the course and quizzes, a PAC/LAN Kidney School Diploma will be awarded to participants.
  • You do not need a computer to attend these calls to qualify for your Kidney School Diploma
  • Please register via phone or fax (you can ask your dialysis Social Worker to fax in the Enrollment Form)
  • Registered PAC Members/SMEs may miss up to 3 calls and still receive their diploma after completing the required module quizzes.
  • There will be an opportunity to make up missed calls in 2015.  Diplomas will be awarded upon completion of all Modules.

To Join for Any Session:

Patients/Care Partners/ Family Members: You can take part in ANY call without enrolling for a diploma (the call information is on the website).  If you would like to attend the wbinar portion of the class, please send an e-mail to abennett@nw2.esrd.net
