As part of our work to increase AV Fistula Rates and Reduce Long Term Catheter Rates, we will be sending out resources to assist you in working with your staff and patients to meet the 2014 CMS Vascular Access Goals.

 The ESRD Network Coordinating Center has developed an interactive flowchart: "It Only Takes a Minute to save your Patient's Lifeline"

This is a flowchart reminder for staff members on how to perform a one minute access check.  It's a PDF with instructions that are available to be printed and used in the dialysis facility,
