For 2014, Five Diamond Reporting is now WEB BASED!
We are excited to announce the launch of an enhanced 5-Diamond Program, which will streamline and improve the reporting process.
Effective January 6, 2014, all users will need to register on the National 5-Diamond Program website as either a program participant or as a "view only" user. Don't forget to indicate that you are a Network 2 facility, so that we will be sent all of your progress and module reports.
As a program participant, you will be able to submit all required reporting on-line and print your certificates once the Network approves your submission. Additionally, you will be able to manage your profile and track the status of each module submission. The Network will continue to award Five Diamond Plaques to those facilities that have completed the required modules for 5 Diamond Status.
Network staff will audit a percentage of completed modules to ensure integrity and compliance within the 5 Diamond Program, and all completed module records must be kept intact and available at the facility for auditing purposes.
For 2014, all modules have been reviewed and updated with new tools and resources. In some cases, measures have been modified to allow for automation. However, the format for each module remains the same: objectives, required activities, tools and resources and optional activities.
To attain Diamond Status, the only mandatory module is the Patient Safety Principles module.
Please note that the Five Diamond Program for Patient Safety is a source of free Staff and Patient education. Conveniently, all modules can be accessed via the web 24/7.
The program consists of 15 Education Modules. Facilities may choose any number of modules to complete, with the Patient Safety Principles module being a mandatory module for all participants each program year.
